Full blood results - help please

Hi i can’t believe it, I went to the hospital at 8 am this morning and my results are all back.... you have to love our NHS sometimes!

Results are back, I have the following results from two weeks ago which is why I think the dr didn’t test them this time

Free T4 16 ( 9-22 )

Tsh 0.05 ( 0.55-4.78)

Free T3 8.2. (3.5-6.5 )

So he will just use those as a guide with the rest of the results maybe?

Any help would be appreciated. I’m falling asleep early, not sleeping well during the night, gaining weight rapidly, despite eating well, my hair is falling out in hand falls and I feel generally horrific.

Your folate and B12 are much too low. Folate should be over mid-range, and B12 at least 550.

I'm afraid the rest of those results mean nothing to me, so cant help you with those. :)

Vit D: 74nmol/L

If that was my result I'd want to push it up into the range recommended by the Vit D Council/Vit D Society, which is 100-150nmol/L. Taking 3,000iu D3 daily along with important cofactors Vit K2-MK7 (90-100mcg) and magnesium should improve your level. Retest after 3 months and come back with new result for comment.

B12 is low at 390ng/L (which is the same as pg/ml). According to an extract from the book, "Could it be B12?" by Sally M. Pacholok:

"We believe that the 'normal' serum B12 threshold needs to be raised from 200 pg/ml to at least 450 pg/ml because deficiencies begin to appear in the cerebrospinal fluid below 550".

"For brain and nervous system health and prevention of disease in older adults, serum B12 levels should be maintained near or above 1000 pg/ml."

Folate is very low and is recommended to be at least half way through range.

I would suggest you consider supplementing with a good quality, bioavailable B Complex such as Thorne Basic B or Igennus Super B at the dose recommended on the pack. this will help raise both B12 and folate levels.

Ferritin is a bit low at 82, it's recommended to be half way through range. You can help raise your level by eating liver regularly, maximum 200g per week due to it's high Vit A content, also liver pate, black pudding, and including lots of iron rich foods in your diet

All the tests within the full blood count/white cell count are within range.

What thyroid meds are you taking? I would imagine combination thyroid hormone replacement of either Levo plus T3 or NDT. Your FT3 is too high and you may benefit from bringing that down into range so a slight reduction looks like it may help.

Thank you so much as always your help is much appreciated. I have slipped from taking my b complex and magnesium recently but both of these are on order again and I will start tomorrow when they arrive, likewise with the Vit d. I am currently taking thyroid s 2 and 3/4 tablets a day I recently dropped to 2 and 1/2 but felt even worse so hopped it again to my usual dose of 2 and 3/4. Is it likely once my vitamin levels are better the reduction on thyroid s could be less noticeable?

Possibly, but they're not terrible results except for folate. However, one thing I forgot to ask last night was when did you take your last dose of NDT before the blood test? It should be 8-12 hours before the blood draw.

Hi seaside Susie, I took my last dose of ndt 1 tablet at 10.30pm and had my bloods drawn at 8am.

So that's the right timing for the test to give the normal level of circulating hormone. I just wanted to check that you hadn't taken it too close to your test which would give a false high FT3.

Thank you, unfortunately not. Going forward, if I don’t feel good with a reduction of medication is there another way to reduce ft3? What are the dangers of it being raised as it is now?

I don't think there's anything else you can do to reduce FT3, reducing your dose is the only way I know. Have you ever tried a combination of Levo plus T3, that suits some people better than NDT as you can fine tune the dose of each hormone to suit yourself.

I don't know how high over range FT3 would need to be, nor for how long, for it to start being a problem but I think this is what could be connected to possible atrial fibrilation and osteoporosis if TSH is suppressed as well - not 100% sure because I haven't kept notes on this so I may be wrong, my memory isn't the best.

I did suggest a combination of levo and t3 to my doctor but he doesn’t believe it would work as well as ndt for me ( ironic as he isn’t able to prescribe ndt - but does support me sourcing my own ) I enquire about this just the other day as I am struggling to source a supply of my current ndt.

Hopefully with the correct vitamin and ferritin / folate levels I will be able to reduce the ndt without issue.

I am still not losing any weight, it’s going up if anything, so you think having a high ft3 could be why?

I've not heard that before. Having low T3 makes weight loss difficult, that's what I've heard.

Thanks, my dear once told me that because my levels are not right, my body will be confused, it doesn’t know if it’s over or under and should be gaining or losing, once my levels were optimal I was losing loads, but I just can’t seem to get back to that place. I have gained 7lb in a few weeks, with no change to diet. I have started taking my b complex, magnesium and Vit d again, but still nothing.

Why not suggest to your doctor that he gives you a trial of Levo and T3 as NDT isn't exactly doing the job.

I might have to, I really didn’t do well on levo though so apprehensive to go back to it. As ndt is in such short supply I don’t see I have an option.

I was taking the same as I am now, I didn’t have bloods tested because stupidly I thought I’d found my sweet spot and it would stay that way. I would say I was optimal between March and August this year. The only things that have changed has been added stress from work in August.
