Thyroid Lab Interpretation Help - Low Frees + Low Cortisol

Hi All,

Firstly, thanks for any advice any of you may have! I was chronically fatigued and in mild pain when a functional MD said according to my blood work they thought I had slight thyroid dysfunction. They started me on NP Thyroid 30 mg and I felt amazing! Energy was back, headaches stopped, cycles became regular. I will also mention that my mom has Hashimotos, which came about after her first pregnancy with my older sister.

I conceived twice in the last year and unfortunately both pregnancies ended in miscarriages. I feel like I am having a super hard time getting back to normal and not quite sure what to do.

I have seen several doctors and they all have given me different advice.

Below are my thyroid results throughout the last year:

8/8/18

TSH: 1.48 (0.45-4.50)

FT3: 3.4 (2.0-4.4)

FT4: 1.31 (0.82-1.77)

RT3: 16.6 (9.2-24.1)

TPO: 8 (0-34)

TG: less than 1.0

FT3/RT3 Ratio: 20.1

10/23/18 (on 1/2 grain of NP Thyroid - FELT OPTIMAL)

TSH: 0.842 (0.45-4.50)

FT3: 4.2 (2.0-4.4)

FT4: 1.07 (0.82-1.77)

RT3: 11.7 (9.2-24.1)

TPO: 7 (0-34)

FT3/RT3 Ratio: 35.9

12/04/18 (1/2 grain of NP; pregnancy #1)

TSH: 1.08 (0.45-4.50)

FT3: 2.6 (2.0-4.4)

FT4: 0.97 (0.82-1.77)

RT3: 13 (9.2-24.1)

FT3/RT3 Ratio: 20

01/17/19 (1/2 grain of NP; conceived pregnancy #2)

TSH: 1.38 (0.45-4.50)

FT3: 3.0 (2.0-4.4)

FT4: 1.12 (0.82-1.77)

02/18/19 (switched to 1 grain of NP; pregnancy #2)

05/23/19 (post miscarriage #2 which happened in April; still on 1 grain of NP Thyroid)

RT3: 12.2 (9.2-24.1)

FT3/RT3 Ratio: 25.4

07/23/19 (1 grain NP Thyroid; 5 mcg of T3)

TSH: 0.067 (0.45-4.50)

FT3: 3 (2.0-4.4)

FT4: 0.97 (0.82-1.77)

RT3: 14.2 (9.2-24.1)

FT3/RT3 Ratio: 21.1

09/10/19 (1/2 grain NP Thyroid)

TSH: 1.84 (0.45-4.50)

FT3: 2.7 (2.0-4.4)

FT4: 0.98 (0.82-1.77)

RT3: 13.3 (9.2-24.1)

FT3/RT3 Ratio: 20.3

TPO: 10

I also had cortisol testing done in August 2019:

Morning: 8.3 (3.7-9.5)

Noon: 1.7 (1.2-3.0)

Evening: 0.6 (0.6-1.9)

Night: 0.5 (0.4-1.0)

I am exhausted 24/7. I don’t feel like I’m running optimally for someone my age (27).

Did I ever have a thyroid issue in first place? Has it been adrenals all along?

I would expect, no one could see anything obviously wrong

Have you had vitamin D, folate, ferritin and B12 tested ?

Your results don't show anything obviously too low

Have you had an ultrasound scan of thyroid? One in five Hashimoto's patients never have raised antibodies.
