Can anyone interpret this Antibodies test result please

Im diagnosed Hypo for 3 years on Levo but never seem to get better, its even worst mostly very tired and much diziness,

GP never suggest to have this TPO antibodies test before, just a friend advised and here the results :

>599 iu/mL 0.00 - 34.00iu/mL

Don’t know If I got Hashi or not? GP always says normall after routine TSH blood test done

Yes, your raised antibodies confirm Hashi's. There is no treatment for Hashi's, it's the resulting hypothyroidism that is treated. However, some members have found that a gluten free diet helps so it's worth trying that for a few months, also supplementing with selenium l-selenomethionine 200mcg daily.

Hashi's can cause gut/absorption problems so it's essential to test

Vit D

B12

Folate

Ferritin

Your dizziness could be low B12, your fatigue could be low Ferritin.

What are your current levels for

TSH

FT4

FT3

Let's see if you are optimally medicated.

GP note on my TPO result : >599 ( 0.00-34.00iu/mL)

Raised as expected in hashimotos thyroiditis ( autoimmune hypothyrodism)

TSH : 1.21 ( 0.27 - 4.2 ),

T3 : 4.01 ( 3.1 - 6.8 )

T4 : 22.1 ( 12 - 22 )

Any result above 34 confirms you have Hashimoto's

Obviously result is significantly higher than that

For full Thyroid evaluation you need TSH, FT4 and FT3 plus both TPO and TG thyroid antibodies tested. Also EXTREMELY important to test vitamin D, folate, ferritin and B12

Low vitamin levels are extremely common, especially with Hashimoto's

Have you had vitamin levels tested?

Recommended on here that all thyroid blood tests should ideally be done as early as possible in morning and fasting. This gives highest TSH, lowest FT4 and most consistent results. (Patient to patient tip, best not mentioned to GP or phlebotomist)

Last Levothyroxine dose should be 24 hours prior to test, (taking delayed dose immediately after blood draw).

Is this how you do your tests?

Private tests are available. Thousands on here forced to do this as NHS often refuses to test FT3 or antibodies or all vitamins

Medichecks Thyroid plus ultra vitamin or Blue Horizon Thyroid plus eleven are the most popular choice. DIY finger prick test or option to pay extra for private blood draw. Both companies often have special offers, Medichecks usually have offers on Thursdays, Blue Horizon its more random

Hashimoto's affects the gut and leads to low stomach acid and then low vitamin levels

Low vitamin levels affect Thyroid hormone working

Poor gut function can lead leaky gut (literally holes in gut wall) this can cause food intolerances. Most common by far is gluten. Dairy is second most common.

According to Izabella Wentz the Thyroid Pharmacist approx 5% with Hashimoto's are coeliac, but over 80% find gluten free diet helps, sometimes significantly. Either due to direct gluten intolerance (no test available) or due to leaky gut and gluten causing molecular mimicry (see Amy Myers link)

Changing to a strictly gluten free diet may help reduce symptoms, help gut heal and slowly lower TPO antibodies

While still eating high gluten diet ask GP for coeliac blood test first or buy test online for under £20, just to rule it out first

Assuming test is negative you can immediately go on strictly gluten free diet

(If test is positive you will need to remain on high gluten diet until endoscopy, maximum 6 weeks wait officially)

Trying gluten free diet for 3-6 months. If no noticeable improvement then reintroduce gluten and see if symptoms get worse

Im current on 100mg Levo but seem to be high with much diziness recently,

Im trying to cut down gluten but its not good enough I guess, not easy..need more time to be 100% gluten free

recent check on Vitamins :

Iron : 26.1 ( 5.8 - 34.5 )

Ferritin : 93.3 ( 13 - 150 )

Folate : 20.10 ( 8.83 - 60.8 )

ViD : 71.80 deficient < 30

Insufficient 30 - 50

B12 > 300 ( 37.5 - 188 )

( too high as I just taken B12 10days before test, just seen recent advised its not good so have to test again soon

So FT4 is 92% in range

But FT3 is only 26.7%

Looking at DIO2 gene test, if you test positive as this can help get T3 prescribed on NHS

Assuming vitamin D in measured in nmol you may find it better getting levels a bit higher around 80nmol or 100nmol

Vitamin D deficiency is frequent in Hashimoto's thyroiditis and treatment of patients with this condition with Vitamin D may slow down the course of development of hypothyroidism and also decrease cardiovascular risks in these patients. Vitamin D measurement and replacement may be critical in these patients.

Evidence of a link between increased level of antithyroid antibodies in hypothyroid patients with HT and 25OHD3 deficiency may suggest that this group is particularly prone to the vitamin D deficiency and can benefit from its alignment.

Presumably you have B12 injections?

Once your on this round a bout you'll never get off. I'm same despite normal, good levels of all bits ect. Think a share in Holland and Barrett may be advised but you'll still be no closer to functioning trust me, me and many others have tried.

Janeyjaney you are right, the symtoms getting worst & its worth to try..

I already cut down gluten quite lots.. but not enough.. it should be 100% for a while to see how’s it goes

I have noticed that I felt slightly better when I eat less carbs, as I am of an age were digestion juices are not at there best so I don't want to waste too much energy trying to digest food. Other than that it most probably is down to the environment and stress factors as these things can zap all my levo in an hour.
