I read somewhere, but can't find it now so am asking you knowledgeable guys, that is optimal to have ft3 levels in upper quarter of range and ft4 levels around middle of range when taking both T3 and T4. Is this true or what should you aim for?

You should aim for feeling well. We all have different needs, so we cannot say that you should aim for this or that point in an arbitary range. Optimal is a feeling, not a number. We need what we need, wherever that takes us in the range.
Well said greygoose . Whatever happened to feeling well over some papers with lab results ??? Lab results are pending on when one dosed when one had the BW etc. It's just a snapshot of that moment . Symptoms are *Cellular* results .
With that said I feel Very Strongly that there is a place for lab results when one does not feel well on current thyroid meds . One needs to know if there is a valid reason for the symptoms and needs to know if some tweaking is needed for hyper/hypo mood .

Totally agree but it’s a shame that we need blood tests. We’re all told that we can’t possible know. For me it’s like recipes/baking. When you first start you have to measure everything out and by the time you’ve baked the same thing 10times you have an eye or a feel for it. My Nan did most of her baking like this but over the years everything including life has become so clinical and humans are loosing their touch for how they feel or their human nature being taken from them by clinicians who’ve been to Uni. Ofcourse THEY need numbers because THEY have no idea what it feels like.
Your so right . It almost feel mechanical with the way we deal with things these days . What ever happened in engaging the patients with conversations like "how are feeling "? Everything is so calculated patients wait in the Dr's waiting room for hours for a 5 minute consultations if your lucky without any interruption's and then your escorted to the door . One is Only left to ponder what was this all about ??? Will we Ever Get Better ????
I know right! There’s no time 😢 I started watching Greys anatomy whilst I was housebound earlier this year and one episode was a storyline of a mum who had brought her son in to the A&E, she had brought him in several times before and no one was listening to her, they were all so busy and although her son was ill there was nothing obvious cropping up. None of the other Dr’s would listen to her concerns. Finally DR Grey said tel me, tell me your concerns. This mother pulls out a list of things she’d googled and instead of rolling her eyes, Dr Grey listened to each one and talked them through and the reasons why they weren’t applicable to her son. But ultimately I think one of the mothers Google’s was correct (but I think it was too late and he died, although I can’t be sure, I watched about 7 seasons and they all blended in to one 😬😂)
I just remember this episode because I was feeling sorry for myself and was fantasising about how different we’d all feel if we had more compassionate medical staff looking after us and working with us. Why are we dismissed so quickly, if GP’s et all are so time poor then why do they not welcome all the research we do, we’re doing all the leg work and no one wants to be well more than us.

When on combination Levo plus T3 (or NDT) then optimal is where you feel best. Some people do just fine with a low FT4 and FT3 above mid way, others need a higher FT4 and FT3 in the upper part of the range. It's very individual. Personally, I've found that I am best with both FT4 and FT3 around 75% through range (I'm on Levo plus T3), but that wouldn't be right for everyone. How you feel will tell you where you need your levels.
