My friend has built up to 4gr TRMan NDT. He experienced some breathlessness soon after starting - and he is a fit man of 51. Reading many other posts, we note that breathlessness is a feature experienced by several people. Can anyone throw some light on why? What is it that causes this effect?

4 grain is a lot. The first thought is that he may be over medicated.
I became very breathless on T4 and only after many months did I find out that this was because I was not converting to T3 and was, in effect, becoming thyrotoxic. The lovely Dr. P. diagnosed the problem. Although NDT has T3 in it, he may not have enough T3. Has he had a full blood test done, as recommended on this site, including T3, as well as all the vitamins, etc.?
When I was on three grains I was over medicated this is perhaps, why. I never had any trouble with my breathing at the time. What does his FT3 read.
Note that one grain of NDT varies according to which NDT you are taking. Unfortunately there doesn't seem to be a scale anywhere to link them. As a matter of fact it's hard to get even the exact amount of T4 and T3 in most NDT pills. For Armor thyroid one grain has 38 mcg of T4 and 9mcg of T3 and each pill weighs 60 mg. It is said that 1grain of Armour NDT has the effect of about 60 mcg of Levo (but nothing to do with the 60 mg weight written on the bottle).
Maybe if everyone on an NDT would give us the proportions of T4 and T3 (for those which are given) we could draw up a scale. Then when someone is on X grains of Y NDT we can make judgements as to wether thats a lot, probably not enough or enough to blow your head off (lol).
It can be low iron (not enough oxygen in blood) or low B12 or overmedication or an allergy to fillers in tablets
