I have been taking levothyroxine successfully (though my T4 is usually high, well out of range and my TSH is usually extremely suppressed) for some 30 years. No GP gas ever said a thing about reverse T3. In very simple terms, what is it, what effects does it have, how do I know if I've got it and what needs to be done?

I doubt many GPs have said anything much about T3 either. This is most likely because such things are far beyond their limited understanding of thyroid disfunction and it's treatments. RT3 is beyond the scope of most endocrinologists too.
Too much Unconverted T4 is putting a brake on your metabolism with a potential underlying consequences of diminishing cellular detox, immune suppression and heart challenges (as the heart only works on T3) in tandem with leptin and insulin resistance... This scenario leaves us easy targets for unwanted guests!
A baseline test for T3 and reverse T3 would be a good starting point.
Then go upstream and eliminate anything that is causing you inflammation such as diet, lifestyle, infectious pathogens, SIBO, SIFO mould or yeast, EMF. . Dr Suzy Cohen has written a good blog article on reverseT3.
I am doing this (under supervision by a supportive Endo for the past year) and have reversed challenging symptoms as the reverse T3 numbers have dropped and T3 has moved in the upper quartile of range. Thankfully after identifying (GI Map Test by Invivo) and removing unwanted guests using herbs.
