My sick note is due to run out after Thursday but I'm still experiencing symptoms post total thyroidectomy. The hospital have booked me in for an appointment next Monday to get me checked over so I'm not sure how I go about getting my sick note extended so it covers Friday onwards?

They are known as fit notes, rather than sick notes nowadays, or more formally, Statement of Fitness for Work. If your GP agrees that you remain unfit to return to work he can provide one for you; it depends what your symptoms are and whether they prevent you from working, or working full time. If they decide your post-surgery health continues to affect your fitness for work, they can issue a fit note for you, on the basis either that you are "not fit for work" , or that you "may be fit for work taking into account the following advice". They can choose the "may be fit for work" option if they think that you are able to do some work even if it not your usual job.
