Need template letter for GP to put me as a named patient for cheaper T3 please

Has anyone got a link for a a letter template to give my GP to be a named patient to get T3 abroad, im sure i have seen one somewhere on the net but can't remember where,, thank you in advance for info if you can help :)

Someone will correct me if I am wrong but I think 'a named-patient' basis is only for medications sourced within the UK. This is information which may be helpful:

A named patient basis prescription is for drugs not licenced in the UK ie Thybon 20mcg T3 or Sanofi Cynomel 25mcg.

I went to my Gp armed with printouts from the web and asked for my prescription of British T3 to be changed to Cytomel (american). It took a lot of pushing from my end and my acknowledgement that Cytomel is not licenced for use in this country although it is used in America and round the rest of the world. Eventually I priced up Cytomel being imported via one of the specialist import companies listed on the Thyroid UK website and compared it directly to the British version. The difference- Cytomel is about £2.50 per tablet (25mcg) and the British one (20mcg) is now over £9 per tablet. The BIG difference is that Cytomel actually works and works really well. My Gp finally agreed to change my prescription and I have been taking Cytomel for about 9 months. I know that the Cytomel is provided on a "named patient" basis but I did not have to do anything other than persuade my GP that I knew what I was asking for and was prepared to take the tiny risk, and I found a local pharmacist who was willing to contact the import company that I found and work with them. I will NEVER go back on British T3, its quality and efficacy is just not stable and as I am on T3 only I am reliant on it to keep living.
