Would a free t4 of 1.16 (.8- 1.8) be considered suboptimal with a free t3 level of 3.1 (2.3-4.2)?

Both your FT4 and FT3 are just under mid-range. So, not likely to be high enough for someone on thyroid hormone replacement to feel well.

But, it's a bit difficult to talk about 'optimal' or 'sub-optimal' when talking about thyroid hormones. 'Optimal' is the level at which you feel well. And, as we're all different, that level will be different for everyone. Do you feel well on those results? If not, plenty of room for an increase in dose.

I think you're taking T3, aren't you? In which case, the FT4 level is of minor importance. The important number is the FT3. :)
