B12 - very top of range, even 900-1000
Folate - at least half way through range
Vit D - according to the Vit D Council 100-150nmol/L (or 40-60ng/ml if that unit of measurement is used)
there isn't really an ideal for B12 - not if using serum B12 as a guide - what really counts is whether you are symptomatic of B12 deficiency - though I realise that evaluating that is going to be really difficult given the overlap with thyroid

supplementing if you have an absorption problem - the most likely reason for B12 being low - can be unhelpful for two reasons
a) if you don't have an absorption problem it can raise your B12 levels where other problems kick in - high serum B12 can cause a reaction that stops B12 getting from your blood to your cell and leave you B12 deficient at the cell level
b) it makes getting a proper reading in tests for absorption problems really difficult.
Serum B12 is a problematic test as it is measuring just one part of the B12 metabolism and isn't a direct measure of what is happening in your cells. There is also a great deal of variation in the levels of B12 in serum that relate to a person being able to function properly which means that serum B12 taken as a single measure will miss 25% of people who are B12 deficient but also pick up 5% who aren't ... so best not to try to treat yourself on the basis of a test result that is known to be dodgy both ways.
