Application form template from RCGP for Online Record Access

This is the type of form that your GP practice should generate for you to fill in for your online access to services including your medical record. This is from the RCGP information pages. Each practice will add in their own name and information so will look slightly different.

Hi

Its not as good as it appears

Yes you can book appointment

Order Meds

View reports blood test ,X Ray results

But does not give across to information your GP has wrote

Aslo you can only view 6 weeks I think it is at a time

It will give coded information if used properly. Unfortunately some GP do not use it properly and can also place date restrictions on access so you cannot see historical information. Different practices do different things, some practices are happy to allow you to see every word and letter. You could contact PatientOnline they can advise. We all need to challenge GP's who decide that they do not agree with you having information that is rightfully yours. I am doing this and raising the issue with PatientOnline

I copy each new page as it becomes available. I know this leaves a paper trail, but enables me to look back as far as I need to.

When my excited lady was setting me up she said there was a date range, so I said I will take as long as it will allow. I got about 10 years test results and a list with dates of reported problems for about 20, no details yet but she is looking into it for me (I was the first request) to see if it can be turned on as when you hit the info button next ti a 'problem' it says go to 'patient section' which implies we should be able to see more. Try asking them to extend the date parameters I know she had to fiddle a bit but she got there in the end

My GP practice deliberately chose to date restrict . They had a meeting about and decided they would give minimum access. 😒

My surgery has not set any online patient access yet. They tell me they have no intention to do so in the near future - but it's a possibility. I thought they had to?

It is a legal requirement to do it. Do not accept this. Please use the information leaflets I have posted. You could contact PatientOnline and they will act for you

Last year I registered with the Patient Access online service: I submitted my postcode to be told that my surgery has not signed up for this. I looked at it again today and it was the same thing! I registered last year, to be informed when they do sign up. I'm still waiting!

if you live outside England there may be other arrangements. I found the service you referred to and it worked okay with my postcode. However this registration is only for basic access not for enhanced access to see medical records and test results.

I think there is more than one company giving patient access to records, and you have to know which one to use for your surgery - perhaps you could register with the other one and see how far that gets you? But if your surgery hasn't signed up, I really don't know how far you will get.

When I registered I was given a bunch of codes I had to use to identify my surgery and me. I still don't have access to any of my personal medical records though.

Yes there are several GP IT systems in use and they all give access to pt records. I posted this in another thread. The codes you were given are initial set up codes for a basic account. To get higher access to your records you need to fill in a new form and sign to say you agree to a few rules. Once this has been approved by your GP the practice then go onto their set up page and alter parameters on your account so the medical record section will be revealed to you. Please try again it really should be very easy.

Oh, I have access to allow me to make appointments and order repeat prescriptions. It's just the medical records access I haven't got. I will be asking next time I'm at the surgery.

I know the company that deals with NHS computers are busy setting up new services in all GPs services

Hi Rose the time scale finished in March 2016. All GP practices had plenty of time to prepare. Every system in England is set up to give the records and if not some may use a hosting service. My OH is part of the NHS IT system

I can't read it clearly but test results are not listed as far as I can see. In the past when we could access our medical records online, although it just consisted of a list, test results were not included in that list. Does the surgery decide what is available to view within the so called medical records ?

Popped into my surgery today, asked for the form and they looked at me like I was speaking a foreign language. 10 mins later a very excited young lady shot into view and said I was the first to ask, merrily ticked every box and I can now see test results for about 10 years and a list of 'problems' going back about 20. If I click on a problem it refers me to 'patient' which seems to be missing so she is going to look into that, that is probably where the form will come in, but its a promising start.

I know know my TSH had been 3.9 (0.2-4.2) or above for at least 10 years 😡😡😡

Result! So pleased you got your record. It is infuriating that information has been withheld and could have made a difference to you. I have similar high TSH and low T4 but GP insists they are "in range" He would never discuss numbers with me I had to interrogate the receptionist but now I have control and can even get graphs on Emis to show him trends. He really hasn't a clue.

I hope all the other people who have posted manage to get access to their records.
