Thy1 or Thy2 result after FNA, please help...

Hi, Im new to thyroid problems so any help is gratefully received. I have a multi nodular goiter & a dominant lump on the left side of my thyroid. I had an ultrasound a couple of months ago, which is U3 & have finally had an appointment with an endo at hospital last week. I am currently waiting to have a biopsy. My endo said lets hope its a Thy1 result, if its a Thy2 result they will want to remove half of my thyroid to be safe. I have been reading up on what Thy1 & Thy2 means & Im confused as from what I can gather Thy1 means they havent enough cells to test & Thy2 means its benign. Please can someone help explain?

JellyQ,

U3 is the classification of your sonographic findings, to determine whether aspiration is necessary.

I can't find where I read it (so can't supply a link) but thought Thy1 is not a diagnosis but indicates the cells taken were insufficient for a diagnosis and will need repeating at a later date.

A Thy2 diagnosis is benign.

I haven't had direct experience of these.

You are right about the Thy1 and Thy2 definitions.

It may be that the size of the nodule means it needs removing even though it is benign. A Thy2 benign result means they will be happy to arrange a hemilobectomy rather than a total thyroidectomy.

Thanks for your reply Clutter, yes thats what I thought too.

Just wondering why my endo is saying he hopes its a Thy1 result? That doesnt make sense to me!?

My lump, which I think is cystic, isnt causing me any problems as regards to swallowing etc so Id rather not have surgery if possible. Is surgery always recommended?

I think your endo may be confused.

I don't know much about cysts but if it isn't secreting hormone or causing swallowing or breathing problems I don't think surgery is always necessary.

Yes I think he must be confused. Ive read & re-read his letter to my GP & thats definitely what hes written! It doesnt make me feel confident.

Ive just received another letter this afternoon which is supposed to be my FNA biopsy appointment but it only says Im booked for an ultra sound scan on it...which Ive already had!

I didnt realise cysts could secrete hormones, but this could explain why Im have symptoms of hypothyroidism, even though apparently my blood tests were normal. As its multi nodular apparently some of the other nodules are solid.

Im just desperate to know what they lumps are & Im feeling so anxious.

If a cyst or nodule secretes hormone you would feel hyperthyroid and would have abnormal blood results.

Are they asking you to have a repeat FNA? My FNA was guided by ultrasound scan.

I feel cold & extremely tired & generally unwell quite a lot lately.

Ive not had a FNA as yet, thats what Im waiting for, but theres no mention of that on todays letter...it just simply says an ultrasound scan (which Ive already had & the result was U3)

But you must have had a fine needle aspiration (FNA) to have the Thy1 or Thy2 result.

Sorry Clutter, just to clarify...I am supposedly booked to have a FNA, the endo sent a letter to my GP to confirm this, along with a copy of this letter for myself...he stated in this letter that he hopes it will be a Thy1 result but if its a Thy2 result he will book me in for surgery.

So, Ive been waiting for the FNA appointment.

In the mean time Ive been reading up on what these results mean (when I get them) & was confused...hence my original post.

The appointment letter has come today & it only mentions another ultra sound scan, no mention of the so called FNA Im supposedly having.

Yes I will do. Im so confused! Its a scary time waiting to find out what a lump in your neck is & the thought of possible surgery :-(

It is very important to further investigations on your neck lump but be comforted that lumps, goitres, nodules and cysts are common in people with thyroid disease and most are benign..

I have spoken to my endo's secretary twice now, as I asked her to double check....she has come back to me saying it is correct, my endo is hoping for a Thy1 result & said that would be fine but if its Thy2 he will put me in for surgery. Im still totally confused!

Your endo is wrong. The BTA Guidelines for the Management of Thyroid Cancer specify what Thy1-5 categories are and the recommended treatment protocols.

Clutter, yes I read them & I agree he is wrong, but I didnt want to argue with his secretary. This is making me so anxious about a situation im already worried about. I have the biopsy date for next Thursday. Im scared stiff.

Discuss it with your endo and tell him you want a second opinion. You shouldn't be having surgery if the result is Thy2.

I will go for the biopsy next week & hope & pray all is benign. Then I will ask for a second opinion should they still want to operate. I was just reading some more & there seems to be still significant statistics of finding carcinomas after resection even though fna showed a benign result. Do you think this could be the reason he will want to operate, as he said "if its Thy2 we will remove the left lobe to be safe"

Thy2 is unequivocally benign. Thy3 is inconclusive. Guidelines recommend surgery for thy3, thy4 and thy5 but not thy2.

I had thy3 but FNA wasn't repeated as the decision was made to remove the nodule to ease breathing and swallowing. The tumour was found to be malignant in pathology and I had completion thyroidectomy 3 months later as risk factors were high.

Im sorry to hear you went through the mill. Must have been such a scary time for you. How are yoi feeling now? Did they find any malignancy in the other half of your thyroid?

The remaining thyroid was benign. I had problems switching from Liothyronine (T3) to Levothyroxine (T4) post RAI but recovered on a combination of T4+T3 and am well now.

Hello again, just posting an update....Ive just received the result from the FNA biopsy I had last week. Its Thy1 so I have to have a repeat biopsy. No idea when that will be. All this hanging on & waiting is scary. MNG feels about the same but the main lump seems to be going up & down unless its my imagination playing tricks on me!?

Clutter: Also my Endos secretary said my endo sends apologies to me that he got the thy1 & thy2 categorisation wrong before in the letter he originally sent me & will be clearing this up in another letter he is going to send! They really shouldnt be making this mistake should they?
