Hi everyone,

I am wondering if the Adrenal Stress Profile by Genova is the same type of salivary cortisol test that an endocrinologist would use to diagnose Cushings/Addisons (as in does it test the same thing using the kind of reference ranges and methods a "normal" lab would?). I am working with my endo from my home country and he suggested I get the salivary cortisol and 24 UFC tests, so I am trying to make sure I'm doing the right ones so they'll be considered legit. ;) Thanks.

Symon,

Only a very small amount of available cortisol is biologically active. A Genova Saliva Stress Test measures this available "active" cortisol (& DHEA) secreted at set times over a 24 hours period. The results will allow you to see any imbalances in the daily circadian pattern so enabling use of correcting supplements to aid your adrenal health. Unfortunately this test is not generally used or recognised by GP's.

Doctors usually conduct a 9am blood test which gives a snap shot of the available bound & unbound cortisol & is used more to assess the ability of the adrenals to produce adequate amounts of cortisol.

To measure free cortisol, urine may be collected over 24 hours, in a test called Urinary Free Cortisol (UFC), which measures excretion of the circulating, biologically active, free cortisol, as too much excretion may be a marker for adrenal dysfunction.. (Cushings ? ? ..)...

You cannot get accurate saliva results while on any kind of medication or supplement that will affect cortisol.

Symon76 From what I have read, as far as I know here in the UK no NHS endo will accept it, they only use their own tests. I don't know about any other country.

Southampton Nhs hospital has used saliva adrenal tests in the past... I dont know if they still do...... Southampton seems like a pretty good hospital.... I go there for asthma and they gave been great.

Thanks for your replies. I was under the impression that the saliva test was pretty standard in Cushing's testing. I know my Canadian endo accepts it although I will need to have a doctor here investigate too since I won't be going home just yet. Maybe if I have a wonky result I can show my GP and she can do her own test if she won't accept the private one?

Hi Symon76,

I did the Genova Saliva test a few months ago. It showed 3 out of 4 readings quite high.

I had a telephone app with my GP who asked me to email the result to her and she would send it on to the NHS Endo, and make a referral.

Good enough service.

The Endo requested a 24 hr UFC which came back as normal.

That was about a month ago and I have to see my GP about it soon.

I suspect the Endo will not follow it up now.

Sounds like you have a good GP at least. I ordered the saliva test, so we'll see how it goes!
