You've been given an ultrasound scan of your thyroid. The detector picks up the strength of sound signal that the tissues like the thyroid return to it. The surgeon has found a nodule (lump) in your thyroid that has the same signal return strength as the normal tissue surrounding it - that's why it's ill-defined. But also in the nodule there are areas (foci) where the signal return is weak (hypoechoic). This means that these areas are less dense than their surroundings.
