An illness, usually a virus, which affects the ability of the thyroid gland to produce hormones for a period of time, sometimes months, but isn't due to dysfunction of the thyroid gland.

Nonthyroidal illness comprises all diseases that are not directly due to thyroidal problems, but can have an impact on the thyroid and pituitary if they are severe enough. Especially if you have to go to hospital severely or critically ill with eg a heart attack, kidney failure, accidental poisoning, liver failure, cancer in advanced state, the pituitary can respond by suppressing or sometimes slightly increasing the TSH level. Free T4 generally remains in the normal range until the Grim Reaper is near, when it falls below the range. Free T3 will be below normal in the "low T3 syndrome of nonthyroidal illness". This is a sparing mechanism rather like hibernation, when the body tries to partly shut down metabolic activity to spare the failing organs. If you recover from the illness, your recovery is accompanied by fluctuations of TSH up and down, below, in and above the range as your recovery waxes and wanes - recovery is not straightforward and short relapses often occur. For this reason thyroid function tests with TSH only are not diagnostic in NTI and FT4 should always accompany them. Actually as some doctor said " you can tell what's wrong with the patient from the end of the bed, and curing that is more important than bothering with thyroid function until they get better".
