Hi My Endo, and I agree ,( experience) does not think this test is reliable enough. I have a more complicated one, a 24 hour urine collection with a low dose cortisone at midnight, this means script ( ie doc) and then blood test at 9am. The GP can do this NHS but if difficult and not seeing an Endo, GP can also do it , as just tests through a private hospital. Not very expensive and very accurate. If you do this shop around, you only need a signed form/letter form the GP. However, as a lot ignorant, easier to find where you want it and ask the lab to give you a filled up form for the GP or any doc to sign. You will need the cortisone though. Urine collection about £50, the blood test a t 9am , not sure of costs but may be GP will do it.

I've never believed a 24 hour urine collection could be accurate. If salivary cortisol shows a spiky response - below the range first thing in the morning, then two samples above the range, then the 4th sample below the range again (for example), or some other spiky pattern, I could well imagine the total daily cortisol result could even out and appear to be normal. With the saliva test, measuring cortisol at four different times in a day can give far more information than a single, composite daily result.

Saliva testing for cortisol is used in lots of research these days. I think it is slowly becoming more acceptable with the medical profession, but the UK is slow to adopt it.

I would have to agree with you humanbean. I cannot see how the daily cortisol reading is helpful as Cortisol goes up and down (so it seems). I know my cortisol is extremely high all day except for one drop late afternoon. Surely 4 samples of saliva taken at specified times is possibly more helpful?

My GP is not familiar with this method, hence me doing a urine collection for 24 hrs. We shall see what the outcome is this week. I would hope for a comparable level of cortisol over the 24 hrs which ever method is used. Otherwise I've just spent £97 approx and I won't know which test is the most accurate or reliable!
