She is feeling weakness. She has lost 3 kg. weight. Must she take medicine now? Is there any medicine available in homeopathy?

My wife is 61 years old. Her pathological test shows TSH(Ultrasensitive), serum by CMIA 20.73 ? Hypo on therapy. What does it mean?
Your wife should see her GP for thyroid hormone replacement. Her TSH is double the range considered necessary for treatment of hypothyroidism. She must be feeling very poorly.
Unfortunately there isn't a homeopathic treatment for overt hypothyroidism. This causes her metabolism to slow down and she's probably extremely fatigued in addition to feeling weak. It will strain her heart if left untreated, and she may experience muscle and joint pain and a host of other symptoms.
It can take up to 18 months to feel well as it can be tricky to find the optimal dose, and each adjustment takes 6-8 weeks before blood can be retested.
Weight loss most definitely can occur in hypothyroidism (which is what the TSH very strongly suggests).
Unfortunately, at least some of that weight loss can be loss of muscle mass and rather than stored energy (e.g. fat). Such muscle loss can be a serious issue.
Further, many people who are hypothyroid also fail to absorb nutrients adequately and suffer the consequences
The phrase "hypo on therapy" suggests that the lab thinks she is taking some form of thyroid hormone - probably levothyroxine - but the rest of your question suggests that she isn't. Can you please clarify.
I am not exactly convinced by homoeopathy, but I do accept that there are some responsible practitioners. If you went to see one or simply read their web sites, you would see that they absolutely state they cannot treat hypothyroidism by their ultra-dilute substances and people MUST take some form of thyroid hormone.
