Had a stroke couple of years ago. Due to excellent physio I am improving remarkably well. Surprisingly the most unpleasant part is I sleep well at about 6 am I am worked up with acute pain in my anus accompanied by this unpleasant tingling sensation around my bowels. It is often alleviated by a poop never a good one. I go back to bed almost on the button it’s a repeat Might not sound a big deal but it's grinding me down. Has been suggested it might be a vagus issue. I keep as active mild but short demanding exercises, a moderate sensible diet.
Since I was a kid I would have anxiety problems which would result in stomach issues.
Since stroke I have had numerous tests, blood, endoscope etc I am showing all clear
I drink lots of water, occasional ginger shots, and a couple of decent cups of coconut water for the electrolytes. I was kept in hospital last year for the tests and had every test under the sun and was told I had no underlying health problems. Good news apparently very fit for my age.
Which of course is aiding my stroke therapy.
Any ideas about the stomach symptoms ? Sounds crazy but under the circumstances I’ve am very lucky but….

If it is proctalgia fugax, I get that. It is a really horrible pain and can come on any time, even wake me up from sleep at any time. It doesn't last long, maybe 10 minutes but it literally has me writhing about while it lasts. There's no point taking a painkiller because it wouldn't have long enough to start working. But I have found sipping some brandy can start to ease the pain within a minute or two. Then it goes completely away and doesn't come back for ages.
Mine has never been connected to a flare up or a bad bowel movement though. But passing wind can often help it, so it could be related to pressure in the rectum from a bowel movement soon to happen in the morning ? I think it's muscles that go into spasm.
Does sound like that but only occurs as soon as I wake up. If I can manage to stay up , don’t forget it can occur at 5am it goes away. It almost feels like an extreme warning I need a poop, but far worse than the obvious.
Yes, it might not be that, then, if it happens at a certain time each morning or most mornings, and only happens after you wake up.
Proctalgia fugax can happen any time night or day, and has certainly wakened me from sleep on a number of occasions. Plus when I get that, there's no sense of needing a bowel movement at the same time. Just pain.
