I get shoulder and upper arm pain and also have IBS but have no idea if they are connected, sorry I can't help.

Hi sweetie... yes!!! I regularly get pain under my right shoulder blade when my colitis flares up. It feels like someone is repeatedly stabbing me through from the inside to my back every couple of minutes and can take my breath away. The way to tell if it's your insides rather than a muscle problem is movement... if it doesn't hurt to move your arm, and the pain goes away if you're walking it's probably referred pain. Also if you press gently onto your stomach (under your breast bone) it's tender and possibly swollen.
My GP prescribed omeprazole (a proton pump inhibitor) to reduce the levels of stomach acid in my system. It works as a preventative over a long period of time and takes a couple of weeks to really settle down. In the meantime I found hot water bottles and paracetamol helpful x
I'd have a chat with your GP and see if they think it might help.

Don't rule out gallstones...
They can radiate into the shoulder and around the back and blades
Have you ever been diagnosed with fibromyalgia? Shoulder pain is common to that ailment. Good idea to check with your Dr. Good luck!
If I were you, I would visit my G P for a physical and possible referrals and X-rays, if possible?
