Chemo Failed
My dad finished
Docetaxel
in November. He went to oncologist today. His PSA went from 0.58 to 2.3. Things he tried: Lupron (on for years before failed) Zytiga (worked for 18 months) Orchidectomy Then the chemo. He is not a candidate for radiation because of a previous cancer.

We are also looking into the clinical trial OR-503 m AB (Cemiplimab/Libtayo) There are so many variables for eligibility in Clinical Trials. He is excluded from many bc of no chemo as of yet. Plus, many require bone metastasis. Thank you for any thoughts!

Needing a bit of inspiration
Nov 2023 ended 5 infusions of Luteium due to low platelets- went on to have 3 quarter strength doses of
Docetaxel
which ended him in the hospital with sepsis due to severe mucositis and aspiration pneumonia. He has been getting stronger and bloodwork is slowly getting better.

Divergent second opinion
He doesn't think that
Docetaxel
is warranted right now unless the genomic test on the biopsy tissue shows high risk markers. He also recommends radiating the prostate plus SBRT to the pelvis mets and to keep chemo as an option for later if needed.

In the UK Olaparib only seems to be available for those testing positive for BRCA. Is this a cost issue that it is not widely available?
I am wondering if this is purely down to cost (Nice will cap certain drugs for us) or whether I will have to fight for it as it is better than say
Docetaxel
. He has not had chemo yet - only radiotherapy at the outset.

PSMA-Addition Trial can give access to Pluvicto for DeNovo or mHSPC
The PSMA-Fore trial recently showed that it iis very beneficial before
docetaxel
chemotherapy in mCRPC. Now a worldwide trial is exploring it in metastatic hormone sensitive PC, including de novo metastatic.

Medical Oncologist suggested waiting for the next two months to see if it’s still rising… Diagnosed 06/2022 Gleason 8/9 Oligo metastatic 3 lytic lesions two resolved Started Firmagon 7/6/2022 Zytiga with Prednisone 7/20/2022 currently still on these meds

Wait or start docetaxel?
MO suggested DORA Clinical trial (everyone gets
docetaxel
, half get radium) but the head of a trial I was previously in at NIH feels that unless a CT or Tc99 scan supports the findings of the PSMA scan I can wait, as long as I have a scan every 3 months.

PSA uptick
Initially treated with
docetaxel
+ ADT. His PSA has been at/near undetected (<0.01-0.02) for several years, but recently there has been an uptick to 0.08 (similar on a recheck, so not lab error). Obviously still low-level, but concerning signal.

What next after Docetaxel...?
On 4/19/23 I was seen by the trial MO to confirm the previous recommendation of 10 sessions of of
Docetaxel
after the new liquid biopsy confirmed the first one with no mutations. Meanwhile, the PSA has risen to 115 before a kidney issue and to about 350 just before Chemo.

Docetaxel not working so stopped
I promised I would update you after my scan to see how the chemo was progressing. Well, the main reason for the chemo was that spots had appeared in my liver. Last week's CT scan showed that the spots have grown "significantly" so the chemo is not working. Also, my liver function blood test results -

Lymph Node Irradiation for a Potential Cure
I only had one
docetaxel
, before catching pneumonia, so chemo was halted by my oncologist, stating the chemo would kill me before the cancer. The one chemo and abiraterone and Lupron shrunk the lymph nodes by 75 percent over time.
