Bipolar Androgen Therapy vs Enzalutamide in Asymptomatic Men With Castration-Resistant Metastatic Prostate Cancer.
The PSA-PFS for enzalutamide increased from 3.8 months after
abiraterone
to 10.9 months after BAT. The PFS2 for BAT→enzalutamide was 28.2 versus 19.6 months for enzalutamide→BAT (HR, 0.44; 95% CI, 0.22 to 0.88; P = .02).

Doctor refuses to test for AR-V7
The doctor says that since hubby progressed on both
Abiraterone
and Xtandi the test is useless. I mentioned re-challenge after chemo but he ignored me. Is this worth seeing another doctor for a second opinion? How valuable is testing for AR-V7 after failure of
Abiraterone
and Xtandi?

Abiraterone
and I am having EBRT on femur. In three weeks I begin Radium 223.

Docetaxel vs. Cabazitaxel--effectiveness?
Last year he tried both
Abiraterone
and Xtandi but PSA is still rising, even after docetaxel. MO wants him to try cabazitaxel next. My question is, is there really that much difference in effect on the cancer? If he's progressing after docetaxel what are the chances cabazitaxel will help?

Apalutamide + Abiraterone combo shows good results in Phase III trial
The trial showed a 30% reduction in disease progression or death for people with mCRPC taking the drug combo (22 vs. 16 months) vs. those taking
abiraterone
(Zytiga) alone: https://www.janssen.com/janssen-presents-results-from-phase-3-acis-study-patients-with-metastaticcastration-resistant-prostate-cancer

Metformin and Androgen Receptor-Axis-Targeted (ARAT) Agents Induce Two PARP-1-Dependent Cell Death Pathways in Androgen-Sensitive PCa Cells.
This is a cell study, but it provides a rationale for using Metformin with
Abiraterone
/Enzalutamide, i.e. androgen receptor-axis-rargeted (ARAT) agents.

Xtandi effectiveness
Next move if Xtandi doesn't work is
Abiraterone
after his Provenge infusions are over.

No intensification - is doc too laid back?
Should I, despite the benign current markers, persuade doc to go for treatment intensification – PARP inhibitor, chemo+
abiraterone
, or whatever? Is there any reasonable chance of delaying time to castration resistance and new mets by doing this?

Loss of appetite
His stage 4, gleason 9, mCRPC is being effectively controlled right now by
abiraterone
, prednisone and lupron. The combination of the lupron and the cannabis seems to particularly difficult in terms of the mental health effects, and he has started considering an orchiectomy in the future.

mCRPC, symptoms testosterone returning, need advice
Using lupron,
abiraterone
and prednisone, been holding his own on this for about 2-3 years. Recent labs were unremarkable and had no indications of cancer spread.

Dangers of AR degradation through ARV-110 etc?
I was just reading on the thread on loss and revival of androgen receptor signaling through the over repression of the AR by use of ADT (Lurpon ) and ADI therapies (
Abiraterone
and Enzalutimide).

Estrogen therapy rediscovered
Before
Abiraterone
was approved, this switch was often done to reduce the PSA level again.
