Piqray and Truqap

Hello,

I am wondering if anyone tried Truqap after having also been treated with Piqray? Prior to Piqray I was on Ibrance for 5 years. I switched to Piqray for 6 months. That was a year and a half ago. I had progression on Piqray and switched to Xeloda. On Xeloda for about 16 months. After progression, switched to Oserdu and am currently on my second cycle. My tumor markers have been climbing consistently for the last 15 months. Scans during that time showed nothing until November 2023. Stayed on Xeloda through the holidays. Scanned again in January and my scans were stable in comparison to scans in November, but tumor markers continued to climb.

We decided to switch course and moved to Orserdu. My tumor markers are still climbing and we will scan in April. I saw my oncologist yesterday. My bloodwork from yesterday showed that my tumor markers are still climbing. She said that we could try Truqap as my last oral option. I asked why it would work when Piqray did not. She said that because it was a 2nd generation of this classification of drug, it may work whereas Piqray did not.

Has anyone been on Truqap after Piqray failed? What are your thoughts?

Hi. There is a Facebook Orserdu group, just so you know.

There is a known fact that Orserdu can make tumor markers climb for months before they come down. I’m not saying that’s what’s happening for you, but I wanted to make sure you knew that. Also, they recommend not scanning for months because it can look like you’ve progressed and get off it too soon. If I’m not mistaken, this med can take up to six months to start working fully.

I am on TRUQAP. I was on all those other drugs too. I started with 200 mg but it gave me too much diarrhea 24/7. I was on it for three weeks, we cut it to three days on and I still had too much diarrhea.

I went off any medication so my body could heal. Then went on 160 mg. The first week. This past week, my cancer 27.29 went down 51 points. Yes! I get blood tests weekly

I am going off this week as I am having cataract surgery. I will still have a blood tests so my oncologist checks I can go on TRUQAP.next week

For the diarrhea my nutritionist suggested Nana flakes. I got it from Amazon. Put it in juice or yogurt up to 3 times a day.

TRUQAP was developed in a lab at MSK.

Thank you. Your experience is encouraging. Piqray was hard for me and I didn't want to go through a similar experience with Truqap. But given what you have shared, I have nothing to lose and everything to gain so I may as well give it a shot! I hope your cataract surgery goes well. That is another thing I will need to do down the road as well. Getting older stinks! But the flip side is, I AM getting older and that is a great thing for all of us with MBC.

I started Orserdu last March after a biopsy showed the ERS1 mutation. Then I had scans the end of January 2024. Unfortunately, more activity was discovered. Thankfully still only in my bones. I had another liquid biopsy and did not show the Pik mutation, so I recently started Afinitor. Good luck.

My gut tells me that because Truqap was only approved on November 23th by the FDA in USA and to my knowledge is not approved in any other major markets yet such as Europe, Uk, Canada etc you likely will not find someone who has done this. You are fortunate that your oncologist is willing to give it a try and I guess the determining factor will be if your insurance company will fund the cost. Although they both target the protein PI3K, Truqap differs in that it also targets 2 other gene mutations in cancer cells.....AKT1, and/or PTEN so?

I too just had the choice of orserdu or Truqap. I’m choosing Truqap.
