BMJ Best Practice Guidelines RLS (UK)

Have you seen this before?

I just got my hubby to look this up (he’s a Dr) and send it to me as this is what my neurologist has mentioned in his letter as what he’s referring to.

It was last updated Jan ‘24, but it’s interesting in some of its guidelines such as mentioning not using DA’s as first line (in some cases) talking about augmentation AND opioids and iron infusion.

Interestingly it mentions RLS-UK, mayo algorithm, IRLSSG, etc

I think it will be useful to take, to those who are having trouble with their GP’s along with NICE etc.

It’s a 61 page PDF and can’t find a way of attaching it here so will email it to Joolsg x

This was last reviewed 16th Feb 2025, peer reviewers Prof K Ray Chaudhuri at Kings, Magdolna Hornyak in Freiburg and Brian Koo at Yale. Will be interesting to see the whole document, as the summary still says "Treatment includes non-pharmacological interventions, dopamine agonists, opioids, benzodiazepines, or neuroleptic agents."

Especially as we have experience here of prof Chaudhuri not following latest practice!🙀

Yup, I decided not to tar the whole review with that brush (yet!) - perhaps someone at the BBC may have nobbled him...🙃

But with Koo on the AASM panel for the new CPG, and Chaudhuri hopefully aware of the new NICE CKS, one would have hoped that their Feb 25 review might lead to another update for the BMJ guideline...

I was referred to Chaudhuri by my local neurologist who didn't know what to do with me . I have done my own research and am now on an opioid. It took a year and I actually spoke to one of his team who surprisingly encouraged the use of opioids and withdrawing Pramipexole. She said that she would follow up in a year. I received a letter saying that the clinic was closing and that I was being referred back to Plymouth . I haven't been, and nobody in Plymouth knows much about rls. Great. Incidentally Chaudhuri charges £1200 for a first consultation.

Was it one of Dr Chaudhuri's team that prescribed taginact as I thought it was a doctor in Plymouth?

Targinact was prescribed by my doctor in Plymouth after an email from Mark Buchfuhrer. My neurologist had previously referred me to London as he was out of his depth.

Can I ask who that doctor was as I keep a list of good doctors as so few will prescribe opioids in case anyone else asks.

Dr C Holland. There was a big feature on Good morning Britain today. This is a major TV channel. A victim of Ropinerole and a supporting doctor gave their opinion of the awful side effects of the drug.
