Buprenorphine Sublingual PLMD

Hi everyone, well after months of no sleep and severe PLMD night and day and a reverse shoulder replacement op, my husband has finally been able to get BUP SUBLINGUAL prescribed. 0.4 twice a day. First 4 days were great, first sleep in forever!! Then the PLMD came back and no sleep for last 2 nights, any ideas of what to do next???

He should try taking 0.4mg 2 hours before bed and the other 0.4mg at bedtime. The half life is 24/25 hours but the largest dose is released over the first 12 hours. He needs the biggest dose in the evening and at night.If taking the doses in the evening only don't help, he may need to increase the dose. The average is 1 -1.5mg so he's below that.

Well we tried that and PLMD the same, day and night. He is also feeling awful during the day since starting Bup. Could that just be side effects at the start? After finally getting it prescribed we hoped it would work! If not back to the drawing board. Maybe we just need to try it for longer.

When you say feeling awful in the day, what are his symptoms?I had debilitating nausea and vomiting for 10 days, but medical cannabis oil sorted it completely.

Then after 4/5 weeks I developed opioid anxiety. 25mg pregabalin sorted that.

If you can solve the side effects, a higher dose may resolve the PLMD.

Average dose is 1 -1.5mg.

Feeling awful is fatigue, anxiety, fed up because nothing seems to work! We will get there eventually. Thank you for all your help. We have to get it on private prescription through GP.

Anxiety can be resolved with small dose pregabalin AND it would help PLMD, help sleep & therefore reduce fatigue. It is frustrating to find the right meds/dose/timing.Good luck.
