Have any of the members on Temgesic experienced a decrease in its effectiveness over time? My husband has had a year of relief from RLS with 0.04mcg Temgesic per day together with a low oxalate diet but the RLS has come back with a vengeance. Would you consider this augmentation? Should we ask our doctor for an increase in the Temgesic or a change to another opioid?

No Temgesic does not Augment ,In the first year it may need an adjustment , Professor John Winkelman and Dr Glen Brooks both say The evidence show it will not require further adjusment and will contunie to work.
Cavet , obviously things can over ride buprenorphine, alcohol can be consumed however you will know if you have to much because it will not work.
Fake sugar also can overide maybe other things they are my 2 experiences.
I have been on buprenorphine coming up to 3 years this year with terrific sucess.Happily I am in possession of many success stories.
Thank you Shumba, I am most grateful. We will do as you suggest and just increase the Temgesic. Unfortunately my husband just refuses to give up the fake sugar!! He doesn't drink alcohol at all but I have wondered about the effect of sweetener on RLS patients. We live in South Africa and are not on facebook but will continue to message through this forum. I couldn't imagine trying to cope with this terrible disease without you all.
PS I would definetly stick with Temgesic and simply add a quarter of a tablet more util legs stop. that way you will know where it needs to be now.I am happy to speak to you on the phone via messenger.

I increased Temgesic each time I had break through symptoms until I found the does that suited me.I take 4x 200mcg tabs sublingual. Two mid evening, two bed time. I have the option of taking a fifth if I occasionally need to.
Dose adjustment in the first year is normal, as others also have said. Just watch out if it becomes an ever increasing dose. But augmentation like with dopamine agonists doesn't seem to occur with opioids for RLS.
