I'm taking one Temgesic 200 mcg sublingual tablet a night. I wondered what dose other people who've been able to get this marvellous drug are on. I asked the neurologist for too little, I think, and will only be able to up the intensity when I see him again next January. Nevertheless, it's by far the most effective drug I've taken and has saved me from The Edge. Does anyone know what the more or less standard dose for RLS is? And if it often titrated? Thanks.

Hi rkatt.

I'm also taking Temgesic. I suffer with severe refractory RLS. I've been taking it since November of last year and I agree, it really helps to keep the symptoms at bay. It's about 75/80% effective for me. I take 400mcgs daily split into two doses. The first one at around 6.30pm and the second around 10.30pm. In addition I also take 100mgs of pregabalin around 9.00pm. (I'm not sure if the pregabalin helps or not and will wean myself off it at some point).

I do still suffer with mild RLS about 3/4 evenings a week but once I move around, the symptoms usually ease within half an hour or so. I also sometimes suffer at night , again about 2/3 times a week, and again the symptoms are mild and usually dissipate within half an hour to an hour and I go back to sleep.

My consultant also allows me to take 100mgs extra if I go on a long car journey as I do still suffer with symptoms (albeit mild) if I'm stuck as a passenger.

Temgesic for me has been a life saver (so far!).

I presume that you are still taking it after 7 months because it works for you. Did you find any side effects? I am in Victoria so I'm hoping I will have no problem getting some to try.

Hi, yes it is working for me. Constipation can be an issue, so I make sure I address this problem. I take Sennalax when needed, drink plenty of water. Eating watermelon and having fibre in my diet helps also. Such as having prunes with my porridge and drinking prune juice.

I also suffer with insomnia and find I get more sleep during the day light hours. So my body clock is very messed up at the moment.

A lot of that is due to being diagnosed with DCIS breast cancer in December 2020.

I have had 3 general anesthetics over the last 6 months, biopsy, bilateral mastectomy and reconstruction surgery. So my body has been through a lot, both emotionally and physically. I am 2.5 weeks out of my most recent surgery and my sleeping routine is a real mess. But the Temgesic is keeping my RLS under control. I have occasionally had to take one extra Temgesic during this time to get me through.

I have suffered with RLS for 50 years and I hope I am never refused Temgesic as it is a life saver for me.

I've been taking 200mcg since November last year. Initially it was a game changer but gradually its effect wore off and I needed to take 2 tablets which then caused me to have a sore head in the morning. The 2 tablets still did not give me a full nights sleep and so I have weaned myself off it and am starting on Ropinirole again. The plan is to take Ropinirole for a couple of months and then switch over to Temgesic again.I'm pleased though to see that Temgesic is working well in others over a longer period than I am experiencing. That is good news.

Ropinirole is a dopamine agonist medication, I was taking Sifrol (Pramipexole) which is also a dopamine agonist medication. I weaned off Sifrol to go onto Temgesic. My experience is that it takes a long time for the effects of a dopamine agonist medication to get out of your system. Any other medications are not truly effective until the withdrawal symptoms are gone. Can take months for the brain to adjust to not having the dopamine agonist medication. I spent two years slowly weaning off Sifrol and had to use Endone (oxycodone) 5mg to help me in the last stages of weaning off.

It took me a year to ramp down from 3.5mg Ropinirole to zero. It was a hard year. I hadn't realised that it took the brain so long to be clear of the DAs. I wonder if it's the same for Temgesic?

I have been taking 2 mg of Buprenorphine for 18 months , however I often only need to take half the dose or a touch more it depends ow much exercise I have done.

I have never ever need to exceed the dose.

Temgesic { 5 hour half life ] is the micro dose version of Buprenorphine [24 hour half-life }

Regarding Buprenorphine Proffessor Winkleman , Earl and Dr Brooks all said that the evidence shows once the dose is right it should never need increasing. this has been the case for me Thank god , I would have gone to Switzerland for a permanent solution if it had not been for Dr Glen Brooks in New York prescribing 2mg Buprenorphine.

For people who Suffer insomnia or pain if you add a small amount of medical marijuana spray it can make a huge difference.

Sorry for jumping in here rkatt.

I noticed you are prescribed 2mg but sometimes take less of Temgesic.

I'm waiting for my Tamgesic script to come through.

What would you think my appropriate dose should be?

My situation is that I have restless limbs 24/7 without my current 30ml (liquid). I take 6x 5mls per day.

I realise we are all different. I was wondering In your experience which dose would be equivalent in Temgesic?

I am a little confused at the different ways it can be taken.

Hi Jane.Temgesic is .02 mcg micro-dose.

there is another lady on this group who takes 2 x 0.2 however you may need more.

Are you are on anything else eg DA's as this will require a much hire dose whilst you wean off .

Temgesic only has a 5 hour half life so if you have symptoms during the day you will need to pop one under your tonge or even half or quater of one it works quickly so you will know when your symptoms stop.

Good idea to have some Zofram handy for the first few days for nausea.

My advice is to break them up and start slow and see how nuch it takes it will adjust over the first week.

Also bare in mind it can cause constipation in some people.
