I read that in 2014 and 2015 , Dr Ferre receieved grants from the RLS Foundation to more closely examine how dopamine and glutamate work in the brain. Apparently he found that low levels of receptors of adenosine, a chemical in the brain that regulates neurochemicals, lead to abnormally high levels of glutamate and dopamine. Consequently thereapies that increase adenosine may help with RLS and Dipyridamole may be one.

Has anyone used Dipyridamole with effect for RLS?
If you put 'dipyridamole' into the search bar you will find a couple of threads I made in relation to my dipyridamole experiment. I pretty comprehensively cover my experience there but am happy to answer any additional questions you might have.
It is worth bearing in mind that I have been through dopamine agonist overuse (over a long period of time), augmentation and withdrawal and it seems that this has a significant impact on the response to medication of many sufferers. The subjects in the study you refer to were all 'treatment naive' (I believe this is the technical term) which means they may have been more responsive to the dipyridamole but it also suggests (though by no means definitely) that their rls was less severe as they had not previously sought treatment for it.
On the US rls forum there are a few people who tried dipyridamole and one or two posted that they thought it actually exacerbated their symptoms while they were taking it.
Of course I had to Google "therapies that increase adenosine". Pretty good article on Adenosine and ATP in the body. The take away is that Adenosine is critical to Sleep regulation and also Inflammation (associated with RLS in some studies). Natural therapies - just 3 - Exercise, Diet (Keto) and some supplements. Many references, on and on!

Is keto best for RLS? I feel maybe not 100 percent keto but higher protein and vegetables and of course basically no processed carbs and lower amounts of grains ...any thoughts here? Wondering if Teri Wahls diet for auto immune problems can help RLS ?
Keto and Paleo are joined at the hip - just really lower carb for keto - higher for Paleo. It is curious that epilepsy responds to Keto diets and also to similar RLS drugs (gabapentin). Keto is just so hard to do - but maybe worth a 2 or 3 week trial. Here's the best keto website...
I tried it, but I was heavily augmenting on a dopamine agonist at the time. Nothing helps when that is happening.
Sergi Ferre had good results in his small cohort.
It is an old and safe drug, so there is little harm in trying.
