preparing for CHC MDT

This is my first post and I’m hoping for some advice on preparing for a MDT meeting for my mum who has PSP (plus depression, diabetes, high BP, cognitive impairment and other conditions. We’ve scored three As (nutrition, mobility and cognition), six Bs and two Cs. She’s in a nursing home (since Dec 21) , hoisted, immobile, double incontinent, communication limited to yes/no and then unreliable (has communication plan through SALT), v high falls risk and choking. Skin integrity is maintained through regular daily treatment throughout the day. She’s on medication for her lack of sleeping at night and also her behaviour/ agitation etc. she has help with all medication and supervision and assistance with meals. DOLs applied for Jan 2021 and doesn’t have capacity. She doesn’t engage in activities at the home. . I’ve followed people on her and know how difficult it is to prove primary health needs and the battle between nursing and care needs so looking for advice really. She has a dedicated RGN and they have electronic care home notes and a detailed care plan. We have her consultant letters and she’s always seeing the older people team or GP. We have no social worker. We have a letter of support from PSPA and I log any medical appointments to demonstrate complexity and regularity of medical advice sought by the home. Advice welcomed

Hi, my husband did manage to get CHC when he was in his 8th year of PSP. We applied 2 years before this and was turned down. It wasn’t an easy process and you need a lot of health professionals behind you. But I wish you all the best. Do let us know the outcome.

thanks for the reply. Glad you managed to get it in the end. Really difficult to tell when is best to apply but she really is dependent on people now for everything :( such a horrid condition.
