Oh Tillie that’s awful everywhere Ken goes now I take a folder with his notes in and hope that they read it I put stickers on the inside page of it including his alergies his medications and stuff about him and hope that they look at it omg
Thi is awful for you the problem is the staff changes and often they don’t know our people and they certainly don’t understand them
I used a form from the NHS, jik.com/CommunicationPasspo..., that I filled out and kept in a red folder. It stayed by dad at all times as he progressed. If I was out a fill-in carer would have access to his info plus who to call etc. The few times he was taken by ambulance to the ER the folder went with the EMT's to be given to the doctor. Even though I would follow the ambulance I didn't always get immediate access to Dad once he arrived at the ER. The doctors who saw the folder said it was an excellent idea, saved them from reading his patient files upon arrival to determine what the overlaying issues were. I can't recall how many doctors upon first seeing dad asked "when did he have a stroke?".
That form looks brilliant. I am always worried that a career or er dept will not understand the little complications. Does anyone know if a similar document exists officially in the US ?
