I was recently diagnosed with prostate cancer. I am 51 with no family history.

PSA 20, 2 lesions on MRI, and biopsy showed one was 3/4 and the other 4/3.

I am pretty much exclusively bottom.

I read here about how gay men should not get a prostatectomy. Whatever little information I have read seem to indicate that radiotherapy is more likely to cause anal/rectal issues. Hence I am feeling very confused. Of course I like the idea of not having surgery( and resultant scars.. I am Asian and tend to scar badly).

I have seen the urologist and will be seeing the radio-onco next week

About 2% of men who have radiation have anal issues that show up after 3 months.

Be sure to ask about whether you have the option for rectal spacer or SpaceOAR to help shield your rectum. It doesn't completely prevent issues, but it does reduce them. A gel is placed between your prostate and rectum under local anesthesia and they may also insert gold targets to help with imaging and beam forming.
