Cousin just had MRI with some pirads 5, having tp biopsy soon,psa 8.3, DRE normal, no symptoms.Is pirads 5 indicative of aggression, biopsy will obviously show diagnosis.

Psa picked up on a requested blood test after much cajoling from me particularly in light of family history.

Bastard disease, got fingers crossed for him.

I was also pirads 5 which my uro said probably indicated aggressive PCa.

PIRADS 5 means high risk for CLINICALLY-SIGNIFICANT cancer, NOT "aggressive" cancer. The biopsy will give a better idea of how aggressive the tumor is likely to be.

I was PIRADS 5, "highly likely" extracapsular extension. But the post-op path did NOT show EPE, and 6 1/2 years after RP there has been no evidence of residual disease. So the finer points of the situation can change over time.

Hi, pd63, below I copied the descriptions of PI-RADS that was included in the report on which the urologist prescribed a biopsy after PSA had increased from 10.4 to 14.2 ng/ml over 2 years:

Prostate Imaging-Reporting and Data System (PIRADS-V2):

* PI-RADS I = Very low likelihood of clinically significant cancer

* PI-RADS II = Low likelihood of clinically significant cancer

* PI-RADS III = Indeterminate

* PI-RADS IV = High likelihood of clinically significant cancer

* PI-RADS V = Very high likelihood of clinically significant cancer

NOTE:

The PI-RADS classification of prostate lesions has been adopted to standardize MRI scan reporting. Current MRI technique and criteria are tailored for detection of clinically significant cancer.

In the report from my first MRI, done 2 years prior to the above, the radiologist stated "IMPRESSION: No suspicious index lesion in the prostate gland", with no statement of any observed PI-RADS, but including a description similar to the above of the PI-RADS levels.

as has been stated, Pirads 5 means high likelihood of cancer, nothing about type. The Gleason score from the biopsy will indicate how “aggressive” it is, with Gleason 5 the highest. Two different 1 to 5 measures.

I was Pirads 5 with a Gleason “4+”, G4 with cribriform. However, my Dechiper score (a genetic test you can add to your biopsy) was only .19, low risk. We still played it safe on treatment plans. Good luck.
