Dad had his 5 years appointment this week and he had surgical removal in 2018 and it was only 0.08 post surgery ( 4+3) and had 37 rounds of radiation and was undetectable for 2 years plus 6 mo the of Lupton. Since 2021, it has fluctuated from 0.05, 0.07, 0.09, 0.07, 0.12, 0.11, 0.09, 0.12 without any treatments. Has been told to just monitor every 6 months and we have confidence in our oncologist. Should we be asking any more questions? Doc thinks this is his pattern of cells that fluctuate as they have for over 3 years now but as long as it doesn't spike, they are content to monitor. Has anyone else had a similar situation? He is fit and well, active and minimal effects from previous treatment. What are our likely next step options?

I am currently putting off irradiation by taking a minimal effective dosage of Bicalutamide. Details and 2+ years of historical data in my thread titled: "An engineer's Bicalutamide maneuvers". Latest PSA: 0.009.

If some Pattern 3 cells are all that was left behind, they may never progress, so watching PSA may be all that's necessary.

Fluctuations at a very low level, slight ever so increase. Remnants left over and maybe some day need attention, but not now as even the best scans wouldn't reveal what/where. So hang on and enjoy the ride!!! Live life and don't suffer too much anxiety until you need to. Because you may never need to...

The 'spike' is what I stay on guard for. My usPSA has been very low stable 0.03X range last two years - my steps are bi-monthly testing, various imaging techniques and blood biopsy testing. My usPSA history is similar to your dads and I also had RP and salvage RT. My third treatment after imaging in Europe at usPSA 0.13 was salvage lymph node surgery that confirmed cancer at para-aortic nodes. All the best!
