My wife who is suffering from gca from last 9 months and is present on 10 mg prednisone . A special inflammation blood test was perform and in report only interleukin 13 came high 10 pg/ml instead of normal limit less than 2.3 attach photo of report
What is important of this finding in gca treatment . Pl guide me if anyone have idea about this .
As I understand this:
"Kasashima et al also showed the significance of the common pathologic features reported in other types of IgG4-RD, including the increased levels of Th2-type cytokine (IL-4, IL-13 and IL-10)-expressing cells and the emergence of lymphoid follicles at affected sites.5, 6 It should be noted that Th2-type cytokines are not involved in the pathogenesis of other types of autoimmune arteritis, such as giant cell arteritis, which is affected by Th1- and Th17-type cytokines."

IL-13 is of no relevance in GCA, the Th-type cytokines that are relevant are Th1- and Th17.
This is likely to be why tocilizumab hasn't dealt with all your wife's symptoms because it doesn't target IL-13, biologics are very specific.
However - that it is IL-13 and IL-10 that are raised also suggests that whatever it is she has, it isn't GCA.
Presumably the doctor who ordered this test will be interpreting the result and advising your wife and your self of the findings? Hoping she receives a treatment which will help her symptoms.
Thanks for your view and experience , on this test and really I surprise when you pointed why IL-6 not included in this test . In my wife case tocilizumab did not work which she took for 3 months .I will ask dr regarding IL-6 in next visit .🙏
