A couple of weeks ago he got a bad back and was in quite a lot of pain that resulted in shooting pains down his legs, now he can barely walk but it improves gradually with exercise. Bilateral hip pain radiating down both thighs into his knees, walking like he can’t move his legs and was woken in severe pain last night, ibuprofen/paracetamol taking the edge off but not stopping the pain. Sound familiar?

It's just it doesn't sound entirely PMR-ish to me - in that often PMR pain doesn't improve much with movement. But a spondyloarthropathy form of inflammatory arthritis does - and it typically starts being painful earlier in the night than PMR does, 1-2am-ish rather than 4-5am-ish.
But bless - they are such sensitive flowers aren't they xxxx
I have at least convinced him to see the GP and work from there, with the knowledge I have re what has happened to me I don’t want him suffering more than he needs to
He says it’s a constant dull ache with occasional nasty pain, I need to read through the other things that it could be, as have been mentioned on this forum so he can go fully armed
I'd tip for myofascial pain syndrome and greater trochanteric pain syndrome (used to be called trochanteric bursitis) - the tight back muscles are irritating or even pinching a nerve that is worse in certain movements or positions which is the sharp pain and the sciatic nerve down the leg pain. Might be other muscle groups invloved.
To be fair it IS how my severe relapses of PMR start off and almost always with a sacroiliac problem (the joint in the pelvis that is about where the baby's dimples are) which is like a knife drilling in at that position.
When it isn't PMR mixing in, my best option is an 800mg flooding dose of ibuprofen and then sitting upright and with back well supported in a chair with a hot water bottle over the painful area. It isn;t an instant solution, it may take a few days, but it usually stops the real agony!
No bladder or bowle problems? Or numbness around the groin region?
Thank you for that, I suggested he read about it and he said that it might be that, hopefully he will discuss the various options with the GP when he sees her in a week or so
Meanwhile I have persuaded him to take painkillers on a very regular basis to keep the pain under control, which it seems to be doing to a good degree
Yes, very familiar indeed. Suggest he consults a doctor for diagnosis. Mine is a combination of weeks of sciatica, osteoarthritis in my hips & a benign tumour at bottom of my spine. But his may be none of those things, these symptoms are common to a few things I think, like myofascial pain syndrome. I can’t fake ibuprofen, it gives me bad stomach pains, &I was warned it can cause stomach bleeds. I still walk, but cannot go as far as normal, it’s painful & I end up in severe pain by the time we reach the car again. The nights are bad. Apart from raised inflammation markers my bloods two weeks ago were fine. Not necessarily connected with PMR or GCA…in my case I had 2 out of 3 before I got PMR.
