I have PMR and due to Steroid induced pulminary embolism, I'm on Warfarin. Antibiotics affect INR levels. Can Covid vaccinations affect INR?

I am pro vaccine, had first Pfizer 24 Feb; 3 days later first ever unprovoked nose bleed for half an hour. Second Pfizer 24 March, then 12 days later 5th April on Easter Monday, massive haemmorrhrage epistaxis bleed 9.00. Tried everything then 111 said 999 so 15.00 ambulance to A&E . BP 200. Could not stem flow. finally 19.00 ENT Dr packed nose after 10 hour flooding. He stopped my Warfarin, in hospital tests four days; he removed inflated packing 8th April, and restarted Warfarin and sent me home. My INR was high 3.8. (target 2.5). Only thing I did different was 2 x vaccinations, so my questions are:

Can the Covid Vaccination put pressure on an already compromised system and induce a nose bleed? and

Can the Covid Vaccine adversly affect INR levels?

I have reported both episodes of epistaxis to the Yellow Card Scheme as have never had a nose bleed before. As ever, grateful in advance for your feedback, Polly

If it did, it sent my husband's level down - target is 2-3, it had been running at 2.3 for ages and then the last two which were after the vaccines, maybe a few days after the first and the day after the second have been 1.7.

The concerns have been about increased clotting so I can't think of any reason the vaccine would have messed things up.

How often is your INR checked?

My husband had his INR checked after first jab (Pfizer) & it was his best ever! Just had his 2nd jab & no reactions, but not due another INR test for 6 weeks. But he banged his elbow (not badly) on the handle of the lawn mower, & was in no pain, but the bleeding under the skin due to warfarin was bad...see photo! Still had no pain, but as he was with nurse for iNR check he showed her his arm & she confirmed it! Of course, if he bleeds at all, it seems never ending before it stops, again due to the warfarin! Sorry, this may not be of help, as he’s not got PMR, but...I hope you will be OK now.

hi Pixix (cute name), that looks truly awful, poor hubby. Thanks for responding. It's always encouraging when kind people chip in with thier experience, makes me feel we're all in this boat together and it's not necesarily the titanic! Maybe get one of those little robot lawn mowers to keep hubby safe from getting knocked up again! as he's bit of a bruiser. best wishes and thanks again, polly.

I think he’s just careless, bashed himself quite often but that was a bigger bruise than usual! Ah yes had a robot lawnmower in a rental home on holiday in Merano, he got excited, came home and it was 1,000£ so he got less excited!! I was amazed he had no pain at all! My brother named me the mad pixie and it stuck! But that name was already taken so I took an alternative that my family often used - Pixix! Glad my story helped a little and yes it just feels like the Titanic at times but isn’t really! Take care, Pixix x

I have been on warfarin for many years. Had Oxford jab and no problems at all as the next day I was at Warfarin clinic and still the same 2.8.

Like Hubby, if a big bruise appears from nowhere or I knock myself. Your hubbies bruise is bruise is impressive as PMRpro says. My pharmacist said to take 1 pinhead sized tablet of Arnica and Arnica cream on outside.

Oh he’s not bothered enough for Arnica, but only as there’s no pain! It was amazing to watch the bruising move gradually down his arm to his hand then see all the different colour changes! But I will remember Arnica next time as I’m sure it’s not good to be quite so black and blue thanks very much!

It is not that there is no pain with the bruise. As I was told, the arnica taken inside helps to break down the small clots than can form from the inside and the cream from the outside.

Below is the proper explanation. I prefer plain language.

A hematoma is benign and is initially in liquid form spread among the tissues including in sacs between tissues where it may coagulate and solidify before blood is reabsorbed into blood vessels....

I had to have an INR test before having the AstraZeneca vaccine to ensure I was within my range. Scheduled test two weeks later was normal.Strange how different practises role out the vaccines.
