I know you shouldn’t use oral
n-saids with pred but can you use the gel to try and relieve specific areas of inflammation , my sternum which I have posted about before is so sore the MRI showed manoscternal joint inflammation but they aren’t doing anything about it , I was promised an injection but when who knows I can’t lay down to sleep anymore it’s so painful proped up at about 45 degrees. What a way to spend your 60th birthday. 😩
I used it on my lower back when on smaller doses of Pred, but PMRpro has advised in the past that so long that its a one-off for acute pain a larger than normal oral dose can be taken. Sure she’ll be along idc course to advise.
Have you tried an ice pack?

Elevation might be difficult!

I’ve had this pain for 2 years, tried all the usual home remedies, ice etc many timed , I have Sapho syndrome also now which is probably the cause of the chest pain , as it also affects my clavicles but the pain in the centre is debilitating . I’m told it mimics a heart attack which I was checked for about a year or so back . I’m only on 4mg of pred so hoping I can try some direct anti-inflam as it’s the only thing now I can think of without proper medical treatment 😩
It’s really worth having a chat with your Consultant or GP (whoever is looking after you) it’s true they don’t like to mix NSAIDS & Prednisolone but under certain circumstances under Medical Supervision Only, there are options.
I had Costochondritis last year & they prescribed the same Anti-Inflammatory l’d previously taken for excessive knee pain Post Chemo.
But it is NOT something you can undertake yourself & the drug l was given is only available on Prescription with the warnings to be mindful of stomach issues......
Again, l have had prescribed Anti-Inflammatory Gel for my knees while on Pred & MTX as a result of the damage caused by the Chemo.
Speak to your GP after the weekend & see what he/she thinks.....

My rheumy does use NSAIDs for acute conditions - and at high dose. I had a week of infusions for a back problem a couple of years ago but he insisted I take a PPI while it was ongoing - that is because it is thought some of the gastric problem is due to the presence of high levels in the blood rather than direct irritation of the stomach lining by the tablets.
At home I use ibuprofen at a flooding dose of 800mg to get the pain under control - and that makes it easier to keep it down. afterwards. You can try with a gel - I don't find they work particularly well but they are absorbed into the blood system too so can be a problem.

I'm sure you will have seen stuff like this - but it may help you think through your approach. Was it better at a higher dose of pred? They list corticosteroids as a management option.
What I do think though is that you need to be insistent to your medics that you can't live like this and need some help. Covid has been a get-out for ignoring chronic patients - but you were ahead of that curve if you have had it for 2 years!
Thanks for the advice I keep leaving messages on the help line but no one gets back to me , my GP is ringing me on Wednesday so I will have a 5 min rant with him . For now I’ll just put up with it , the most annoying part is feeling miserable and unsociable my daughters have made a great effort for my birthday this weekend and I just don’t feel up to it , but I will paint on the smile and get on with it .
