Can Pudendal neuralgia cause ovary pain as main symptom?

Hello,

TL;DR: Mainly pain in ovaries, always worse right before periods, can it still be PN? Or does that make it more likely that it is something else happening.

First thought my adenomyosis had gotten worsen, but specialist said it looked very good and normal. Found out about PN just a few days ago. It didn't seem unlikely in my current situation where I've been bedbound for a few years now for other reasons.

I'm mostly seeing that PN is supposed to hurt in different parts of the genitals, but for me it mainly hurts a lot in my ovaries or that area, also hip bone, when I am upright or straining myself in some way or laying in specific positions. Is this common for PN, or would this indicate I most likely do not have it? I do also have very intense pain in my clitoris and sometimes in my groin, but usually only a few days a month. The ovaries and in between them is where it tends to hurt always if I strain myself. If I lay down and do not strain myself it is usually okay unless I'm a few days away from my period, then it hurts anyway.

My pain always gets a lot worse closer to my period (it is not typical period cramps, I've had those since I was 12 and am 30 now.) The ovary pain is so bad I've taken morphine a couple times now which is new for me. Is this also unlike PN? or can PN typically get worse/flare up close to periods?

I'm wondering if just pelvic floor dysfunction might be more likely for me. My muscles in my whole body are very weak because of the bedbound situation. Any input is appreciated. Trying to diagnose myself has proven very difficult, and getting to the hospital for different types of evaluations isn't easy when bedbound. The last one was a couple weeks ago with the endo specialist. Trying different remedies and lifestyle changes for the sake of potential endo hasn't helped even a little despite going at it for months, so I agree with the specialist that there is most likely some other issue at play.

it doesn’t sound typical of pudendal neuralgia .

HiI doesn't sound like PN to me. You can get pain around your ovaries but you would also have sharp shooting pain up your bum/vagina and sometimes abdominals.

And you would have the pain all the time or everyday at least rather than around your monthlys. There's a book called "a headache in your pelvis" that may have some info in it that could possibly help. I would highly suggest looking up videos on the keto diet and ovaries on you tube you'll no doubt find some interesting info. I have PN really bad and the only thing that has helped me and I've had surgeries you name it is doing this diet.

Thank you Jo, that is I guess good to know I most likely don't have it. But it makes me wonder what's actually going on instead since the gynecologist can't find anything wrong.

I have been on a very strict ketogenic diet for 3 years so I agree it's a good step towards health. Also cut out dairy and all processed foods, etc. Unfortunately it doesn't seem to do anything for this specific issue.
