Can the pain from pudendal neuralgia entrapment be sited in middle of buttock? Female.

I have had PNE for over a year, diagnosed by Baronowski in London. The pain became manageable but a week ago, after exercising, the nerve seems to be irritated in middle of buttock - I can feel a sharp, needling sensation there. The pain then radiated inwards to the groin. Any ideas how to improve that? And has anyone had a successful injection - with which British doctor? Thank you!

Specialist pelvic floor physio can work great, I have levator-ani syndrome and I have definitely benefited with decrease in pain.

Try baths with Epsom salts and take magnesium glycinate supplements.It could be your pubo-rectalis and pubo-coccygeal muscles are spasming,

Pain patches applied to that buttock might help to.

When the muscles spasm, they irritate the nerves.

How many injections have you had? What was injected into you and where was the injection site.

The pudendal nerve gets entrapped most often between the sacrotuberous and sacrosphinous ligaments in the ischial spine. 77% of entrapments are found between or on these ligaments.

I’ve had the surgery and it saved my life. Injections don’t last if it’s true entrapment. Decompression surgery is needed and the sooner the better.

Thank you for your detailed reply - much appreciated! How was your trapped nerve diagnosed? Know that if I avoid hard surfaces, any heat and certain way of stretching my leg, my pain settles. Think I managed to do all of the above in a short space of time, hence the flare up. It’s taking a long time to resettle now. Hope you manage to have a very happy Christmas!

I had seven nerve blocks over a 3 year period all to no avail. Surgery proved the entrapment.

If three CT guided nerve blocks done into the nerve roots via the buttock with both anaesthetic and 30-40mg steroid don’t settle the nerve, they suggest surgery. These three blocks should be done one month apart in a sequence.

My own doctors gas-lighted me. I have fought for my life. I HAD TO hire a nurse consultant because no doctor would advocate for my surgery.

The surgery wasn't available in Canada in 2011. I needed to go to Minnesota. My government would not support it without the go ahead of a neurosurgeon.

The nurse I had to hire and pay pleaded to a top neurosurgeon to send me there because I was near death from uncontrolled pain and sleep deprivation. I truly am lucky to be alive. Doctors in British Columbia did not care. I was neglected.

Dear Konagirl60 can you tell me how your decompression surgery got done and the tests prior also is it best to ask for a referral to a neurosurgeon, as I'm interested in checking for entrapment and also have you had sacral modulation' implant for pain as both I'm interested in and tests prior to determine if nerve entrapment, I had pudendal nerve block vaginal , with no effect at all but was not Fluro guided. Surgery proved the seriousness of my entrapment. Conservative medicines did not relieve my pain nor did nerve blocks. I had the transgluteal approach through my right buttock. The pudendal nerve and part of the inferior rectal nerve were released from the ligaments they were stuck to.

My surgery was procured with me hiring a registered nurse consultant who advocated to a top neurosurgeon for me. No doctor where I live in Canada would support it. It was done 3.5 years after it became entrapped so I’ll never fully heal.
