Afternoon all

looking for some more advice. As previously said been feeling feeling terrible for about a year.

Doctors thought that it was my low folate as my level was low. Got that level up to late teens and my b12 is high nearly 1500.

my ferratin is 29 which is on the low side but my iron study has come back fine

serum iron 27

Uibc. 48

Iron binding 36%

Doctors said because the iron study is ok the low ferratin is irrelevant

Any advice pls

Hi Lee,

Please fill in your bio a bit. I have no idea if you are in Australia, USA, Belgium and all have different health systems.

Plus, people have an idea how long you have had hydroxycobalamin or methylcobalamin injections for.

You do not mention symptoms but you just say you feel terrible. So, these vague symptoms do not really help with building a clinic picture. A better way is :-

1) Severe fatigue/exhaustion.

2) Poor cognitive function.

3) Headaches/migraines.

4) Poor appetite or whatever.

Still able to work or not etc. This links into stamina.

So, we always go on symptoms and your B12 should be high with injections. So, why did they even bother to re-test ? It is a waste of money. The British Haematological Society states that once treatment had commenced no further testing.

In order to test your blood, they need your consent. People often give implied consent by offering their arm. You can say, Do not check my B12, please write it on the form but you can check my other bloods. Which you have checked which ones before. Now, if they do check your B12 and you have explicitly told them, it is in writing and they have done, they are in very serious trouble.

So, no, I am not happy with a ferritin of 29 mg/L for you. Up around 50 - 60 mg/L. I noted haemochromatosis flashed up - do you have concerns in this area ? It is good a full iron panel was taken.

Live in the uk been on b12 injections for 15 years Unsure if I have PA due to the intristic factor test being unreliable.

Terrible fatigue

Tinitus

Weakness

Neuropathy

And rls

Still able to work

Low ferritin normal iron study

Okay so get that print out (hard copy) of time and date of low B12 with number from 15 years. That is confirmation. Diagnosis

Now, the way around this is you write P.A/B12D because you are being treated. The treatment is the same for both.

Do you know if you had IF test - intrinsic factor test ?

How often do you have injections ?

Is there any family history of Autoimmune disease ? Tested for Coeliac Disease ? Thyroid. See when you put stuff in order, doctors can cut through 💩, have a better idea of what’s what. Better decision making such as ordering tests, investigations, referrals.

Neurological involvement so Every other Day injections are required. Your symptoms are neurological.

Do you Self Inject ?

Restless legs syndrome ??

haemochromatosis is a blood disorder - genetic - you would know about it. A person can over supplement with iron but you need yours slightly higher to activate Hydroxycobalamin plus the folate issue.

A question about ranges: my last test for ferritin showed a reading of 33. The range is 11-306. It seems to me that the range is so large that any reading below mid-range could well indicate deficiency. I realized I was anaemic from symptoms and the bottom eye lid test earlier in the year despite my within-range test result. I began taking ferrous fumarate and have steadily improved. I think I'm one of the 20% with anaemia rather than peripheral neuropathy. I now have a useful and established scepticism about blood test results and ranges indicating "normality", let alone medical pronouncements about them.
