You can order cyano from biosenseclinic.com in Canada. I have used them for several years and they are reasonable in price and reliable. A 30ml vial runs about $20 shipped. Do not keep a multidose vial longer than 30 days after first draw. Cyano is the most common form of B12 and works for most people. I used to pay high prices for Methyl and I found it to work no better, and in some ways worse than cyano and hydroxo. There has been lots of talk about how methyl is the only "naturally" occurring form of B12. That is a crock. If it come in a vial from a manufacturer, it has been synthesized. The cyanide scare tactic is also false. There is only a tiny amount in cyano and the molecules are bound very tightly. You will not get cyanide poisoning from using cyano. I have been injecting daily for over two years.

Yes, it does. I worked in clinical healthcare and always follow the quality control procedures recommended by the United States Pharmacopeia (USP) and the manufacturer for multi-dose vials. If a multi-dose has been opened or accessed (e.g., needle-punctured) the vial should be dated and discarded within 28 days unless the manufacturer specifies a different (shorter or longer) date for that opened vial.
