Use google chrome to access the site -you will then get translation . Then look under prices . The prices given are the charges to health insurance companies . Best to phone them . The Dutch all speak good English.

Thank you everyone. I live in the USA and trying to see if there is just a cash fee. Not sure our insurance will cover it
