If you're not in UK, may be worth trying to track down any national/regional b12 guidelines for B12 deficiency.
I have found the B12 shot has greatly changed my periods, which could help with fertility. however I do take extra folic acid and iron too which also seem to help.

I also wondered if you had had recent results for folate, ferritin (or other iron results) and full blood count (FBC), known as Complete Blood Count (CBC) in USA.
Do you get copies of all your blood test results? Access to medical records/test results can be invaluable in knowing what investigations have been carried out.
I learnt to always get copies of blood test results after being told that everything was normal and then finding abnormal and borderline results on the copies. These days I only accept being told things are "normal" if I've seen results myself.

PA is an auto-immune disease. Having one auto-immune condition may increase chances of developing another.
I hope doctors have excluded possibility of other auto-immune conditions especially those that may influence fertility in your case. I have read posts in past on forum where thyroid disease, diabetes and coeliac disease are mentioned.
Thyroid Disease
It's not uncommon for people on this forum to also have thyroid problems.
I would suggest putting any thyroid results on Thyroid UK forum on HU. In UK, doctors often only test TSH which will not give a full picture of thyroid function.

Due to pattern of my symptoms, I asked my doctors to exclude possibility of lupus and Hughes Syndrome (also known as Antiphospholipid Syndrome, APS).
A search for "B12 deficiency infertility " had links including one that linked high homocysteine with potential infertility. High homocysteine levels may result from B12 deficiency.
