immediately after you have an injection your serum B12 levels will be off the scale but they will drop - very rapidly at first but as the rate is dependent on the amount of B12 in your blood it slows over time. So it would be more correct to say that by the time you retested your levels your B12 was dropping down to 300. The most likely explanation for the higher levels this time is that you didn't leave as much time between the injection and test as you generally do.

The rate at which B12 is removed is a very individual thing.

After loading shots serum B12 normal range can be very misleading - some people respond to the elevated serum B12 levels immediately after the shot in a way that tries to stop the B12 going from their blood to the cells. They are okay if their levels are above a tipping point where enough trickles through but once it goes below this they start to develop all the signs of B12 deficiency even though their serum B12 levels are very high. What matters after loading shots is your symptoms not your B12 levels, which means regular testing after loading shots isn't necessarily that useful.

Do fasting or non fasting may effect the b12 test level?

I'm not medically trained but I don't think so for B12, however it may affect Folate levels if taken after breakfast.

Hopefully someone more knowledgeable will clarify Jotk9

I wouldn't worry to much on the level of your B12 blood result Lilly60, mine have been just over 2000 in the past, around a year ago & still at that time i felt I needed more due to my symptoms, but this Novembers result showed it had gone down to 727 but for the past two years now I have not felt right in myself & even though I have been having a monthly injection I still am having balance issues my memory is very poor & sometimes I experience breathing problems . I saw the doctor last week & he said I am not deficient but when I got a copy of my results I began researching their meanings & my MVC was high at 97.8 which shows I have Macrocytic anaemia & when I read up about it ,it explained all the symptoms I have been having including Ataxia= gait, balance & co-ordination problems which says is caused by B12 & folate deficiency, & even though the doctor agreed that my MVC blood test does relate to macrocytic anaemia & also my MCH result was high at 34.2 he still didn't see a problem. :-( & I left the room saying it looks as though I will have to sort my own health problems out then. So it's not always the B12 level that indicates an issue, check out your other results. I have had Pernicious anaemia for 19yrs now & these past couple of years I have felt very similar to how I did when I was first diagnosed. I am happy to say I have taken the matter into my own hands after plenty of research & yesterday was the first time I self injected & I will be doing so on alternate days until I have improvement in these awful symptoms. x

I know what you mean about just getting on with things because trying to get the doctors to listen can be like pulling teeth.. but there comes a time where just getting on with it isn't good enough & you realise it needs to be fixed before any lasting damage is done. Have you requested to see a haematologist & a neurologist ? I am waiting to see those now, but I think doctor only agreed to shut me up, but at least I may get answers. In the meantime I am going to continue to self inject & see how I improve. Good tidings also to you & your family Lilly. x x
