I’m on my second recurrence and currently on carbo/gemcitabine.. ive had a reaction on last three cycles each one being harder than the last.. my question is has this happened to anyone else and , if so what was was the outcome? It’s been mentioned they may slow carb infusion over Two hour period or split doses ie gemcitabine one day and then carbo another day? Anyone experienced this! Thank you all

Thank you for answering. I have two more treatments to go, been suggested we try again at a much slower rate or split my days ie carbo one day gemcitabine another. Decision to be made when my oncologist back from holiday

I became allergic to carboplatin when on carbo/gemcitibine for my 2nd recurrence. My treatment was slowed down a lot and I managed to complete the course. The following year my oncologist wanted to try carboplatin alone for my 3rd recurrence so I had a desensitisation regime and managed the full course without any issues. I think having the infusions via a portacath helped too as my veins didn’t get irritated.

Will mention desensitisation, they did give me steroids and anti histamine thru my port and i had to wait half hour in the hopes it would work. Will discuss my plan with oncologist on 12th, really hoping i can push thru as treatment is working xx take care

The desensitisation involved splitting the treatment into 3 or 4, the first bit was very dilute, given very slowly, and each subsequent bit of the dose a bit stronger but still given much more slowly than is usual, with steroids and antihistamines to take at home for a couple of days before the treatment as well as intravenously on the day. Hope all goes well for you. x

Hi, I’m also on my 2nd recurrence & being treated with Carboplatin as a single agent. After carbo for my 1st recurrence I started to develop a rash on my arms & neck but nothing major. The rash only lasted about an hour then disappeared.

For this round of treatment, it’s been given over 2 hours with additional steroids & antihistamines and this has worked wonders - no rash at all. As a separate issue, unfortunately my CA125 is on the increase during treatment so my oncologist feels I’m probably platinum resistant now & has arranged a CT scan before I see him again and will be changing my treatment if my scan shows progression but as far as the carboplatin goes, the allergy protocol has worked for me.

Good luck xx

Im hoping by slowing down the infusion next visit i will be able to finish treatment, my last ca 125 showed a decrease from 907 to 402 which is brilliant! Waiting for a call from my cns, thank you for replying

Hi. I had a severe reaction to carbo on my third cycle of my first recurrence. I had a grade 3 anaphalactic shock. I can no longer have carbo and had to swop to cisplatin.

in reply toKatmal-UK

How was Cisplatin for you? I will have to have this next time, as I too had anaphylactic shock with Carboplatin during my 2nd recurrence

Xx

I found it harsher as I the side effects were harder but it’s doable .

I had Carbo/taxol 6 rounds in 2008. No issues. Last year, when I was rediagnosed, they tried Carbo/gem. I had a reaction (hive) to the Carbo. The Onco switched me to Gem only (not overly successful for me). He mentioned slow-infusion Carbo as a future possibility. Good luck to you.

Hi I am at exact same stage as you. I had bad reaction on second infusion of Carbo so my oncologist arranged to have it slowly dripped. However on the third try even with that I had a severe reaction and completely passed out, woke up to severe vomiting with hot prickly sensation around mouth and on hands and feet and it had to be stopped. Since then I have had it as desensitised where they start with very diluted bag and then gradually increase it over the next 4 bags. This has worked fine twice now. I still get a small amount of prickly sensation around the mouth and hands but otherwise have tolerated it. Two more to go now. I hope your oncologist will sort this out for you and you can get to the end particularly as it seems to be having good results for you. I've just had my scan so I'm waiting to find out if its working for me. Good Luck!! X

T hanks for reply! I had sane reaction but didn’t pass out.. i fell i to a half sleep and could feel my ears irritating and itching round my mouth, my nurse woke me to see if i was ok they carried on with treatment but my hands and feet started to burn b and swell so it was stopped, discussing whether to change to cisplatin, but i would like to see if i can tolerate a much slower pace , speaking with onc on 12 , fingers crossed it will be sorted. Take care

My oncologist talked to me about stopping the carbo and starting again on cysplatin but then you literally have to start again from the beginning so if you’re already on cycle 3/4 that’s not great?! So I’m glad I have peresevered but I’m very anxious at the start of each cycle just in case it still happens again!!

oh blimey, the thought of having to start all over again is not what I want to do🤦‍♀️🤦‍♀️ Someone on here has mentioned desensitisation which I’m gonna discuss with my oncologist, yes I will be anxious at my next treatment, this blimmin journey is no fun, take care

I've had cisplatin as an overnight treatment once and carbo on the desensitisation protocol three times now. You get the carbo in 4 bags 90mins each. The whole day takes about 8 hours if you're getting another drug, plus flushes, premeds, etc. Cisplatin used to be done as an overnight treatment but these days on the day chemo unit but needs two pumps as you have fluid at the same time. Both require special bookings with the scheduler as they are non-standard in the sense that they are more resource intensive.

Hi. I am about to start my fifth of six cycles of Caelyx with Carboplatin . All was fine until ten minutes to the end of cycle 4 when I developed an allergic reaction to the Carboplatin...my twelfth treatment with this drug. I was given Piritin which stopped the intense itching, and the carbo was not finished because the sudden back pain was so bad. My oncologist is saying I should try again with extra steroids and a slower rate of infusion for the Carboplatin so we will see how that goes. I have also developed a spotty rash from week two on the last two cycles but no other longer term effects so far. Fingers crossed That I can get through these last two cycles as CA125 down to 600 from 6000 so far.

Best wishes for your future treatment.

Thank you so much .. i to an hoping my oncologist will be able to do a desenitisation programme for me as i only have two more cycles to go and it’s working for me, whilst the reactions are alarming, im hoping by slowing infusion down s much as they can i will get thru it, fingers crossed !
