I had a cortisone shot in my shoulder and had immediate relief but three days later the pain was back.
Im wondering if i done to much by drilling holes in a steel plate on the 3rd day or if its not bursa but a bad rotator cuff, anyone else had this experience?

You do have to be careful about not overdoing things when you have had steroids as they do make you feel so much better it is easy to overstretch yourself without realising. Have you ever had cortisone in that shoulder before? There is now evidence that steroid injections into tendons can make tendon rupture more likely (though I think if the injection was right into the synovial space there isn't that risk).
I have the same problem but the steroid injection didn't do much still on tramadol for the pain just waiting now for a ultrasound to see what the damage is. my biggest worry now is does it need surgery as being self employed its my busiest time coming up.
hope all goes well just keep on to your doctor, if there like mine they will eventually get something done.
