My wife's(GFR 13) started the transplant process in November 2023. Her identical twin has been through extensive testing for donor acceptability. She is a 6 for 6 match. Her GFR based on creatinine is over 90 and all of her tests have neen positive. Yesterday, she got one of her final blood test results showing Cytatin C at 1.34 which (Googling ) is end stage renal. We are terrified that she, for some reason, now has serious kidney disease and obviously not a donor candidate. Waiting response from her care team but we are sleepless. Comments please.

The egfr calculation is based on both creatinine and cystatine c . Only creatinine can also be used for egfr calculation and was the norm till very recently. I have not seen any egfr calculation based only on cystatine c.
Googling can sometime be misleading. Hope you get better news from your care team soon. My very best wishes🙏
Thanks so much. I went to kidney.org/professionals/KD... with her creatinine and cystatin values and it was no CKD. Feeling much better while waiting care team official word.

Cystatin 1.34 is not end stage. Looks more like stage 2 kidney disease. kidney.org/professionals/KD...
