You are invited to complete a transportation survey if you are a dialysis patient who uses either Medicaid or paratransit (transportation for people with disabilities provided by local governments or state funded programs) transportation services as your mode of transportation for your dialysis treatments.

This online survey is a part of a research study to examine non-emergency medical transportation services for dialysis patients and to identify whether dialysis patients face barriers when accessing those services.

This is an online anonymous survey about the transportation services that you use for your dialysis or transplant related medical appointments. There is no confidential or identifiable information collected in this study. The survey has 11 questions and it may take 10 minutes of your time to complete the survey. There is no compensation provided to participate in this study. You must be 18 years of age or older to take part in this research study.
