Best to speak to your doctor, but I know that when my father (whose GFR at the time was around 27) had long term antibiotics it went down to about 17 but that was after being on a high dose for many weeks.Once they'd finished his GFR bounced back up to 40 on his next kidney function test.
So any effect was temporary and I'm guessing that because the infection was resolved it actually caused an improvement afterwards.
I have Stage 4 and have taken it several times in the past 4 months for cellulitis and a lung infection. Didn't the Dr. who prescribed it know you have Stage 4 CKD or are you taking leftover meds that weren't prescribed for you? That is never a good idea.
If you don't trust your Dr. find a new one.

My renal doctor said the prescription the primary ordered was to high a
Dosage for a kidney patient. The primary had all my records and labs from the kidney doctor. This was her mistake that caused me drastic results.
Please check with your kidney doctor before taking any antibiotics.
So sorry same thing happened to me. Primary doctor prescribed wrong dose of BP. meds.
My Nephologist will allow antibiotics...but at half dose.. .be very careful
