Speak with your dermatologist. The actinic keratosis can be also be treated with a zap of liquid nitrogen. See if your case qualifies.
Welcome to the community. This page from the AAD (aad.org/public/diseases/ski... list many options but only your medical team can tell whether a specific treatment is suitable or not.
Your derm and nephrologist need to work together to determine how much of the topical is absorbed through the skin and for how long. Might turn out to be negligible and not harmful to the kidneys.
