I am 41 year old man and for the past 4 years my eGFR has been between 61-84. Last test showed it at 77 and I also had a 24 hour urine creatnine clearence which showed my creatnine very high at 2.84 on a 0.5-2.15 scale. My nephrologist who is a RN (registered nurse) didnt know what it meant for it to be so high and said she will ask a doctor. She doesnt have answers to many of my questions and always says she will ask another doctor ( an MD). Anyway, all my other numbers came out good. No protein in urine, albumin, BUN all good. No other diseases, no hepatitis, no immune disorders. Just high creatinine in urine and in blood (its how they calculate eGFR by the creatinine in blood). She said I have CKD stage 2 ( because my eGFR is 77 ) but my understanding is that there has to be other factors besides eGFR to say its CKD, like protein in urine, which I dont have.
Does anyone know what causes creatinine in urine to be higher?
Edit: I dont eat alot of meat or protein and havent really exercised in almost a year. I dont take any medication. Thank you!

Creatinine is horrible for estimating GFR, they can send the same blood to 3 different labs and get 3 completely different values. Even when they do it right, you could get a blood test in the morning that says you have ckd, and take blood in the afternoon that says you are normal just because of hydration levels alone. You are right, you are not stage 2 unless you have some other indication (protein in urine, water retention, etc.). Get a cystatin-c test if you have not already, it is more accurate for calculating eGFR.
Hello db79. I looked up other causes of high creatinine and found this information about high creatinine levels and some other different causes. I’ve attached the link from Healthline below if you wish to read it over. It does list dehydration and urinary infections as possible triggers also.
I know you mentioned that you don’t consume very much protein, but what about protein bars or shakes that may be fortified with creatine? I’m not sure, but I just wonder if any of those could possibly pose a problem as well.

Hello once again db79. I apologize for this, but I was just wondering if it is at all possible that your urine creatinine test scale may have been somehow confused with a blood creatinine scale. I’ve noticed that each test has a very different range scale (and also a different “normal” for men than for women). Also, urine creatinine testing, I believe, is performed over a 24-hour period. This is just a suggestion, but you may want to insist on more testing to be done (just my opinion). Good luck! 🙂👍
Thank you for your replies. No worries. I did both, the blood and the 24 hr urine. Theyre supposed to be done at the same time. I have high creatinine in both blood and urine. But no protein and everything else looks normal.
